Psychiatric Nurse Practitioner - WellConnect / SAGE

DESC•Seattle, WA
•$160,836 - $177,576

About The Position

DESC is hiring a Psychiatric Nurse Practitioner to provide evidence-based mental health and substance use care to clients experiencing severe mental illness and often co-occurring substance use disorders. The role is housing outreach-based, serving clients within DESC’s supportive housing programs and working alongside multidisciplinary behavioral health teams. The position allocates 0.4 FTE (two days per week) to the WellConnect intensive behavioral-health-within-housing start-up team, modeled on assertive community treatment programs and focused on clients with high behavioral health needs in DESC supportive housing. The remaining 0.6 FTE (three days per week) is with the SAGE (Support, Advocacy, Growth & Employment) team, DESC’s largest outpatient longitudinal behavioral health program. SAGE is clinic- and housing-based, but this position will predominantly engage with and see clients in supportive housing. Tuesday and Wednesday are highly preferred for the WellConnect workdays. The Psychiatric Nurse Practitioner will collaborate with psychiatric nurses, case managers, social workers, substance use disorder professionals, peer specialists, support staff, and other psychiatric and medical providers. The WellConnect program is funded for 14 months, with anticipated long-term sustainability after the pilot period. This is a full-time position with an office day shift and Saturday and Sunday off. The position is supervised by the Outpatient Psychiatric Provider Supervisor and is represented by SEIU Healthcare 1199NW.
