About The Position

Seniority Healthcare is seeking a full-time Psychiatric Nurse Practitioner to support multiple senior living and community-based facilities through a hybrid care model. This role includes on-site clinical coverage across designated Long Island communities and remote telehealth support for additional New York and New Jersey locations. The position requires flexibility in scheduling, with both on-site and remote coverage, depending on patient volume, operational demands, and evolving program needs. On-site coverage will primarily support communities in Plainview and Rockville Centre. Remote telehealth coverage will support communities in New York and North Jersey. This role is ideal for a clinician who is comfortable working across multiple sites and modalities, and is open to adapting as the organization expands services, including potential future development of a PCM/primary care model.

Requirements

  • Board Certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC).
  • Active Nurse Practitioner license in New York, in good standing.
  • Active Nurse Practitioner license in New Jersey, in good standing, to support telehealth services.
  • Ability to practice independently within state scope of practice and organizational policy.
  • Strong documentation, clinical assessment, and care coordination skills.
  • Comfort with technology and telehealth-based care delivery.

Nice To Haves

  • Experienced Nurse Practitioner, ideally with prior experience in senior-focused or community-based care.
  • Prior experience working in a multi-site and/or hybrid on-site/telehealth care model.
  • Familiarity with structured clinical documentation workflows and evolving care delivery models, including CoCM, PCM, and evaluation/management related processes if applicable.
  • Experience in senior living, assisted living, community-based care, or similar settings.
  • Ability to work effectively with both clinical and non-clinical site partners.

Responsibilities

  • Provide psychiatric diagnostic evaluations and management for residents/patients in assigned communities, both on-site and via telehealth.
  • Conduct comprehensive and problem-focused evaluations and develop individualized treatment plans.
  • Order, review, and interpret diagnostic testing as appropriate.
  • Document clinical encounters accurately, thoroughly, and in a timely manner within designated clinical documentation platforms and workflows.
  • Coordinate care with community staff, internal clinical team members, and other providers to ensure continuity and quality of care across multiple sites.
  • Support care delivery in an evolving hybrid model, including adapting to operational changes, varying visit volume, and new workflow development.
  • Collaborate in the build-out and refinement of expanding clinical programs, including potential Collaborative Care Model, PCM, and other psychiatric care model growth.
  • Work effectively with future support roles, such as behavioral health care managers, medical assistants or care coordinators, as workflows continue to develop.
  • Maintain compliance with all applicable New York and New Jersey regulations, licensure requirements, and organizational standards of care.
  • Demonstrate responsiveness, professionalism, and strong follow-through in both remote and in-person care settings.
