About The Position

Grace Community Care & Homes seeks a part-time Psychiatric Mental Health Nurse Practitioner (PMHNP) to support planned mental-health and substance-use services in New Jersey. The role includes psychiatric evaluations, prescribing, medication follow-ups, and coordination with the medical director and clinical team. Start date and clinical duties depend on applicable program approvals and payer credentialing. Remote responsibilities include assessing patients, developing treatment plans, prescribing within New Jersey authority, monitoring medication response and side effects, managing refills and patient questions, maintaining electronic records, and participating in virtual case reviews. Addiction-treatment duties depend on demonstrated qualifications and assigned scope. Planned IOP and residential assignments require a separate coverage agreement; any necessary on-site work will be disclosed before assignment. The initial planning target is approximately five appointments per clinical day. Hours will reflect visit length, documentation, and follow-up work. Applicants should submit a CV, certifications, patient ages served, availability, relevant experience, and requested hourly rate. Final regulatory eligibility will be verified before appointment or service delivery. Grace Community Care & Homes is an equal opportunity employer and provides reasonable accommodations.
