About The Position

This position involves providing nursing assessments, direct treatment, and management of health conditions for patients through various means including in-person, telehealth, and phone consultations. The role requires obtaining patient history, performing examinations, ordering and interpreting diagnostic studies, and providing preventive health assessments. Responsibilities also include counseling patients and their families, documenting healthcare delivery, assisting in office locations with medication administration, maintaining medication stock, and coordinating medical documentation. The role requires effective communication with various community agencies and personnel, informing of important information efficiently, and assisting in the preparation of treatment plans and discharge summaries. Additionally, the position involves consulting with community groups and providing in-service education.

Requirements

  • Master’s degree and credentials from the Kansas Board of Nursing as a nurse practitioner.
  • Two years of experience in a supervisory team setting.
  • Must pass KBI, DCF child & adult abuse check, motor vehicle screen, and background checks.
  • If the position requires the use of a personal vehicle for work purposes, the employee must have reliable, insured transportation that is properly registered and compliant with all applicable state and local requirements.
  • Strong communication, problem-solving skills, and organizational skills.
  • Ability to work collaboratively in a team environment.
  • Ability to work independently in the community.
  • Proficient with computers and word processing.
  • Ability to operate a vehicle as part of routine job functions.
  • Ability to use a computer and software as part of routine job functions.
  • Ability to lift and carry up to 25 lbs as part of routine job functions.

Responsibilities

  • Nursing assessments of patients seen in person, telehealth, and by phone, including assessment of side effects and communication of essential information to the medical team.
  • Obtain patient history and perform assessment via observation, interview, and examination; order, perform, and interpret diagnostic studies; perform preventive health assessments, screening, and patient care.
  • Provide direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources.
  • Counsel and educate patients and their families/caregivers concerning preventative health, treatment options, and community resources.
  • Document delivery of health care and nursing processes in accordance with specific specialty area standards and nursing policy.
  • Assist in one or more office locations, including administration of IM medication, sample medication, and other medication; maintain medication stock in compliance with Federal, State, and Center guidelines and requirements; administer Indigent Medication programs.
  • Communicate effectively with personnel of community agencies, laboratories, pharmacies, medical clinics, courts, etc.
  • Inform of important information quickly and efficiently.
  • Maintain and coordinate medical documentation in compliance with Federal, State, and Center guidelines and requirements.
  • Assist in the preparation of timely treatment plans, discharge summaries, and utilize review documents, including computer records and prescription records.
  • Consult with community groups and provide in-service education.
  • Perform other duties as assigned by your supervisor or the Executive Director.
