About The Position

This is a high-impact, full-time opportunity for a Bilingual (English/Spanish) Psychiatric Mental Health Nurse Practitioner (PMHNP) who is Medicaid credentialed to practice across a blended care model—combining inpatient and outpatient hospital psychiatry with dedicated time in our outpatient Neurology & Psychiatry clinic in Lower Westchester. Requirements

  • Bilingual (English/Spanish) required
  • Active Medicaid credentialing required
  • Board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
  • Active and unrestricted Nurse Practitioner license
  • Strong foundation in psychiatric assessment, diagnosis, and psychopharmacology
  • Ability to work effectively across both hospital and outpatient settings
  • Excellent communication, clinical judgment, and documentation skills

Responsibilities

  • Conduct comprehensive psychiatric evaluations across inpatient, hospital outpatient, and clinic settings
  • Diagnose and manage a wide range of psychiatric conditions in medically complex populations
  • Provide medication management, including initiation, titration, and ongoing monitoring
  • Participate in inpatient rounds and multidisciplinary treatment planning
  • Provide crisis intervention, suicide risk assessments, and acute stabilization
  • Coordinate transitions of care, including discharge planning and outpatient follow-up
  • Deliver longitudinal care within the outpatient Neurology & Psychiatry clinic
  • Collaborate with neurology providers on neuropsychiatric and cognitive disorders
  • Educate patients and families on diagnoses, treatment plans, and medications
  • Maintain accurate and timely documentation within EMR systems
  • Participate in clinical meetings and quality improvement initiatives
