About The Position

Telos Health Systems is seeking experienced Psychiatric Mental Health Nurse Practitioners (PMHNPs) to provide care to geriatric residents in skilled nursing and/or assisted living settings. These in-house “SNF-ists” support residents’ quality of life and well-being while helping reduce return hospital stays. The coverage area is Norfolk, Virginia. The role is available part-time or full-time, with a flexible schedule of two to five days per week. Compensation is offered through a salary or wRVU model; the posting gives a good-faith annual salary estimate of $140,000–$160,000 for a full-time PMHNP seeing 75 visits per week. A $5,000 sign-on bonus is offered. The SNF-ist serves as a leading provider of internal medicine services at a skilled nursing facility and as a day-to-day clinical leader. The practitioner collaborates with the facility’s interdisciplinary team and physicians, overseeing care for short- and long-term residents with support from nursing staff and guidance from medical directors and attending physicians. The role includes assessments, diagnosis, treatment, medication prescribing, care planning, referrals, communication with other clinicians, documentation, and CPT coding. Telos describes a collaborative environment focused on career development and meaningful work. Employees receive a flexible work-life-balance schedule, a hybrid model combining in-person care with virtual charting, company-provided equipment, and mileage reimbursement under company policy. Professional support includes occurrence-based malpractice insurance with $1 million/$3 million coverage limits, an annual $1,000 allowance for continuing education, DEA registration, and licensure reimbursement, plus additional professional development support. Full-time employees working at least 30 hours per week may access medical, prescription, dental, vision, retirement, voluntary insurance, paid time off, and paid holidays, subject to the stated eligibility period.
