About The Position

Merakey is seeking a part-time Psychiatric Mental Health Nurse Practitioner (PMHNP) to support our Assertive Community Treatment (ACT) team in Stroudsburg, PA. This role involves providing comprehensive mental health care to patients living with chronic mental illness within the community. The PMHNP will work collaboratively with a multidisciplinary team to diagnose and manage mental health concerns, including prescribing and monitoring medications, and facilitating hospital admissions when necessary. The ACT team's goal is to provide community care and support individuals who have not been successful with traditional mental health services.

Requirements

• Certified Nurse Practitioner, PMHNP with prescriptive authority.
• Minimum one year of experience in community mental health.
• Licensed in State of Pennsylvania.
• Valid Child Abuse, State Criminal and FBI Fingerprint clearances.
• Master’s degree in nursing with a focus on psychiatric-mental health.
• Ability to work collaboratively in a multidisciplinary team environment.
• Excellent communication and interpersonal skills to effectively engage with patients and colleagues.
• Familiarity with electronic health records (EHR) systems and healthcare technology.

Nice To Haves

• Experience with integrated health preferred.
• Experience in crisis intervention, urgent care, or emergency psychiatric settings is preferred.

Responsibilities

• Conducting psychiatric patient evaluations to determine medication needs.
• Documenting evaluations and interactions with the patient in the electronic health record.
• Prescribing pharmacologic therapies including controlled substances.
• Monitoring patient progress and making necessary changes to medications.
• Facilitating direct patient admissions into hospitals when needed.
• Participating in weekly multidisciplinary team meetings to develop coordinated treatment plans.
