About The Position

The Psychiatric Mental Health Nurse Practitioner (PMH-NP) functions under the clinical supervision of licensed physicians, provides diagnostic and therapeutic health care to patients in Skilled Nursing, Memory Care, Assisted Living, Independent Living environments, in our Fox Valley and Green Bay service areas. This is a full-time position with a 1.0 FTE commitment, operating Monday through Friday between the hours of 7:00 am and 5:00 pm.

Requirements

  • Current, unrestricted licensure as a Nurse Practitioner or Physician Assistant.
  • Active DEA registration and current BLS certification.
  • Demonstrated experience conducting comprehensive psychiatric evaluations in accordance with established standards of care.
  • Proven ability to develop individualized psychiatric treatment plans and provide thoughtful, evidence-based medication management.
  • Strong clinical judgment, assessment skills, and a patient-centered approach to care.

Responsibilities

  • Provide comprehensive psychiatric care to adult and geriatric patient populations across a variety of clinical and care settings.
  • Manage an average daily caseload of approximately 16 patients, with travel between multiple facility locations as needed.
  • Conduct thorough psychiatric evaluations, including assessment of mental health status, cognitive functioning, and presenting concerns.
  • Evaluate, diagnose, and manage patients with a range of psychiatric conditions, behavioral health concerns, and dementia-related disorders.
  • Establish and clearly document appropriate medical and psychiatric diagnoses based on clinical findings, history, and assessment.
  • Order and interpret appropriate diagnostic laboratory and radiologic testing when clinically indicated to support evaluation and treatment.
  • Develop, implement, and regularly evaluate individualized, evidence-based plans of care in collaboration with patients, families, and the interdisciplinary care team.
  • Prescribe and manage medications in accordance with clinical guidelines, patient needs, and applicable standards of care.
  • Collaborate closely with the supervising physician and interdisciplinary team, incorporating clinical guidance and feedback to support high-quality patient outcomes.
  • Participate actively in collaborative clinical meetings, case discussions, and care coordination activities.
  • Maintain timely, accurate, and comprehensive clinical documentation and billing in accordance with organizational and regulatory requirements.
  • Apply sound clinical judgment and critical-thinking skills when navigating complex, urgent, and emergent patient situations, while appropriately following established protocols and adapting interventions to meet individual patient needs.
  • Promote continuity of care through effective communication and coordination with facility staff, healthcare providers, patients, and families.
  • Maintain a professional, patient-centered approach while working across multiple facilities and adapting to varied clinical environments.
  • Perform other duties and responsibilities as assigned in support of patient care and organizational needs.
