About The Position

Merakey is seeking a Nurse Practitioner to join its team in Hammond, Louisiana. The Psychiatric Nurse Practitioner delivers mental health care to individuals living with chronic mental illness as part of a multidisciplinary team in a community-based setting. The role includes assessing, diagnosing, and managing patients’ psychiatric needs, prescribing and monitoring medication, and coordinating treatment with other care team members. Community-based services may require traveling to patient locations with team support.

Requirements

• Have an active driver's license in the state of Louisiana.
• Be willing to work in a community-based setting.
• Be a board-certified or board-eligible psychiatrist, a medical psychologist, or an advanced practice registered nurse (APRN) with a specialty in adult mental health in Louisiana.

Responsibilities

• Conduct comprehensive psychiatric evaluations to determine medication needs.
• Document patient assessments and interactions in the electronic health record (EHR).
• Prescribe and manage pharmacologic treatments, including controlled substances.
• Monitor patient progress and adjust medications as necessary.
• Facilitate direct patient admissions to hospitals when appropriate.
• Participate in weekly multidisciplinary team meetings to develop coordinated treatment plans.
• Collaborate with psychiatrists, registered nurses, and other care team members.
• Provide community-based services, including traveling to patient locations with team support.
