About The Position

The Advanced Practice Nurse will be responsible for conducting visits to patients in accordance with local, state and federal regulations. They will communicate visit findings to designated members of the interdisciplinary team, facilitate access to appropriate support models of care, and consult with other members of the interdisciplinary team to contribute to the patient's plan of care. The role also involves preparing and maintaining accurate patient records, charts, and documents, and assisting in the development of clinical practice guidelines/standards in support of quality care as needed. Ensuring collaboration with patients, caregivers, medical directors, and members of the IDT is also a key responsibility.

Requirements

  • Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
  • Nurse Practitioner's license in the state of operations
  • PMHNP certification
  • 1+ years experience working within Family, General Practice, Geriatric care or related field
  • Commitment to ongoing professional development and clinical excellence.
  • Must have a valid driver's license and a reliable source of transportation
  • Must be willing to travel locally

Nice To Haves

  • Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required.

Responsibilities
