About The Position
Mindify Wellness and Care is seeking multiple New Jersey-licensed Psychiatric Mental Health Nurse Practitioners (PMHNP-BC) to deliver psychiatric services. These services will be provided onsite at partner adult day centers located in Sewell and Pennsauken, New Jersey, as well as through telehealth. Providers have the flexibility to work at one or both centers, depending on their availability and the centers' specific needs. Onsite presence is a requirement for this role.
Requirements
- Must currently hold all required New Jersey credentials and prescribing registrations.
- Master’s or doctoral degree from an accredited Psychiatric Mental Health Nurse Practitioner program
- Active New Jersey RN license
- Active New Jersey Advanced Practice Nurse (APN) certification
- Current PMHNP-BC certification
- Active DEA registration
- Active New Jersey Controlled Dangerous Substances (CDS) registration
- At least one year of experience as a psychiatric mental health nurse practitioner
- Ability to provide onsite services in Sewell and/or Pennsauken and telehealth services to patients located in New Jersey

Responsibilities
- Conduct initial psychiatric evaluations and follow-up visits
- Diagnose behavioral health conditions using DSM-5-TR criteria
- Prescribe and manage psychiatric medications
- Develop and update treatment plans
- Monitor patient progress, medication effectiveness, and side effects
- Educate patients and families about diagnoses, medications, and treatment options
- Complete accurate and timely documentation in the EMR
- Collaborate with therapists, social workers, primary care providers, and other members of the treatment team
- Conduct risk assessments and crisis evaluations when clinically appropriate
- Follow HIPAA requirements, clinical standards, and organizational policies
