About The Position

EverCare Mobile Health is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to support residents in long-term and residential care settings. The role involves partnering with facilities to provide consistent, evidence-based mental health services aimed at improving patient outcomes and quality of life. The model emphasizes continuity of care and allows providers to maximize meaningful patient interactions. Care goes beyond medication management, focusing on building relationships, supporting emotional well-being, and helping patients maintain independence and stability. EverCare Mobile Health serves independent living, assisted living, skilled nursing, memory care, and group home environments, enhancing collaboration, improving diagnostics, and reducing caregiver burden by integrating into facility care teams.

Requirements

• Active PMHNP or PMHCNS license
• Idaho licensure
• Strong communication and collaboration skills
• Self-starter with ability to work independently
• EHR proficiency

Responsibilities

• Conduct psychiatric evaluations, medication management, and therapy
• Develop treatment plans and collaborate with care teams and families
• Document care accurately in EHR
• Participate in interdisciplinary coordination
