About The Position

CareOne in partnership with BridgeCare Provider Group is seeking a compassionate and experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide comprehensive medical care to residents in a skilled nursing facility (SNF). The PMHNP will evaluate and manage psychiatric and mental health conditions, develop individualized treatment plans, coordinate care with the interdisciplinary team, and promote quality outcomes for a medically complex, primarily older adult population. Travel will be required - no overnights unless necessary for trainings.

Requirements

• Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program
• MUST be PMHNP certified
• Must have a Nurse Practitioner's license in the state of operations
• Must have 2+ years' experience working within mental health, geriatric/adult populations
• Must have a valid driver's license and a reliable source of transportation
• Must be willing to travel

Nice To Haves

• Experience in a nursing home, hospital, assisted living or acute rehab space is strongly PREFERRED
• Commitment to ongoing professional development and clinical excellence

Responsibilities

• Conduct visits to patients in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team.
• Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).
• Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts and documents.
• Assist in development of clinical practice guidelines/standards in support of quality care as needed.
• Ensure collaboration with patients, caregivers, medical directors and members of IDT.
