About The Position

Provide in-person psychotherapy and behavioral health services to older adults and medically complex patients in Skilled Nursing Facilities (SNFs) and Assisted Living communities. The position offers independent clinical practice, flexible scheduling, consistent patient volume through established referral relationships, and administrative and operational support. Providers work with an established multidisciplinary team and build ongoing relationships with residents and their families.

Requirements

Active and unrestricted California NP license.
Master's degree or doctoral degree in an applicable discipline.
Ability to provide compassionate, patient-centered care.
Strong communication and documentation skills.
Comfort providing services in nursing home and long-term care settings.
Only fully licensed providers will be considered; applicants with limited permits, associate licenses, LMSW, MHC-LP, provisional licenses, or supervised/pre-licensure status are not eligible.

Nice To Haves

Experience working with geriatric populations.
Experience in Skilled Nursing Facilities, rehabilitation centers, long-term care, or assisted living settings.
Interest in interdisciplinary and integrated healthcare.

Responsibilities

Provide in-person psychotherapy and behavioral health services in Skilled Nursing Facilities and Assisted Living communities.
Conduct diagnostic assessments and develop treatment plans.
Treat depression, anxiety, adjustment disorders, grief, trauma, and other behavioral health concerns common among geriatric populations.
Maintain timely and accurate clinical documentation.
Collaborate with interdisciplinary care teams to improve patient outcomes.
Build therapeutic relationships with residents and their families.
