About The Position

OnePeak Medical is redefining primary care through a personalized, integrative model that blends traditional and functional medicine. Our mission is to treat the root causes of illness and empower patients to live healthier, more vibrant lives. With 20 clinics across Oregon and growing, we’re seeking a passionate Mental Health Nurse Practitioner to join our primary care team and make a difference through holistic, functional, and patient-centered care. This role is a telehealth role supporting our Oregon clinics.

Requirements

Candidates based in the Pacific Time Zone.
Active Oregon license as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) or eligibility to obtain.
Board certification as a PMHNP is required.
Strong interest in functional, lifestyle, and integrative approaches to mental health.
Excellent communication and interpersonal skills with a patient-centered mindset.
Ability to work collaboratively in a multidisciplinary team environment.
Proficiency in using electronic health records (NextGen experience preferred).

Nice To Haves

Minimum of 2 years of clinical experience preferred
NextGen experience preferred

Responsibilities

Provide comprehensive psychiatric evaluations and ongoing medication management for patients across the lifespan.
Diagnose and treat a range of mental health conditions, including anxiety, depression, mood disorders, and ADHD.
Develop individualized treatment plans that may include medication, therapy recommendations, and lifestyle interventions.
Collaborate with primary care providers, health coaches, and behavioral health professionals to support integrated, whole-person care.
Educate and empower patients on mental health, wellness strategies, and preventive care.
Maintain accurate and timely documentation in the EHR (NextGen).
Participate in team meetings, clinical collaboration, and quality improvement initiatives.
