About The Position

At At Home Harmony, we believe seniors deserve better than the fragmented, clinic-bound care model. We're building something different: an integrated, home-based care model that brings primary care, psychiatric care, long-term care pharmacy, advanced dementia support, and personal care directly to the people who need it most — in the place they know best. Based in Richmond and growing fast across Virginia and beyond, we're looking for a compassionate, sharp, and adaptable PMH/APP who wants to make a real, visible difference in the lives of older adults navigating psychiatric conditions and complex chronic illness. This role is anchored in psychiatric care, but you'll also provide primary care to a medically complex population — so if you love variety and want your clinical skills to matter every single day, this is for you.

Requirements

  • 3+ years of experience as a provider working with geriatric patients
  • Strong clinical experience in geriatrics, psychiatry, dementia care, cognitive assessment, and chronic disease management
  • A genuine passion for relationship-centered care with older adults and their families
  • The energy to thrive in a fast-paced, high-volume environment without sacrificing quality
  • Sharp time management, documentation, and organizational instincts
  • Confident, healthy boundaries with patients, families, caregivers, and partners
  • A talent for navigating hard conversations — goals of care, caregiver stress, end-of-life planning — with warmth and clarity
  • Comfort with telehealth, EMRs, and AI-enabled documentation tools
  • Current, unrestricted VA license as a PMH/APP
  • Current national board certification
  • Current BLS
  • Active prescriptive authority and DEA registration
  • Valid driver's license with a clean driving record

Responsibilities

  • Deliver care where patients actually live — in their homes or assisted living facilities
  • Diagnose, manage, and treat complex chronic illness with real clinical autonomy
  • Interpret test data and coordinate follow-up care to keep patients on track
  • Empower patients and families with the knowledge to manage their own health
  • Lead compassionate, honest conversations about end-of-life care and goals of care
