Requirements
- Current State of Connecticut certification and/or license as an Advanced Practice Registered Nurse.
- Active DEA controlled substance registration and an active Connecticut controlled substance registration.
- DATA 2000 Suboxone waiver, or willing to obtain one.
Responsibilities
- Provides Mental Health and Addiction treatment services to patients in Middletown, CT clinic.
- Provides continuity of care by arranging follow-up appointments, coordinating care with a team of social workers and counselors.
- Provides care services including, but not limited to, initial evaluation, physical examination, and medication management, coordinating care with other providers, ordering labs and EKGs and obtaining information on adverse events.
- Provides continuity of care by arranging follow-up appointments.
- Administers medications, including but not limited to Esketamine (Spravato).
- Obtains prior authorization for services, provides assessments and other testing as needed.
- Prepares reports and participates in agency quality assurance activities.
