Requirements

Bachelor's Degree in Physical Therapy
Current NYS Physical Therapy Registration/Licensure
A valid NYS Driver's License and ability to travel between sites

Nice To Haves

1-3 years of related experience preferred

Responsibilities

Evaluate and assess individuals on an ongoing basis to develop personalized, meaningful treatment goals.
Implement therapeutic interventions based on assessment findings, helping individuals enhance mobility, function, and quality of life.
Educate individuals, families, and team members on treatment plans, therapy methods, injury prevention, positioning, range of motion, and more.
Collaborate closely with interdisciplinary teams including educators and healthcare professionals ensuring alignment with each Individual Program Plan (IPP) or Individual Education Plan (IEP).
Maintain accurate and timely documentation in accordance with program and regulatory requirements.
Support departmental operations, including equipment needs, adaptation, and orientation of new staff.
Participate in agency-wide initiatives that advance our mission and enhance the well-being of the people we serve.
