About The Position

As a NP at Horizon, you will provide initial psychiatric examinations for individuals admitted to mental health, rehabilitation and recovery programs and provide follow up medication management services. You will participate in clinical case conference and case reviews to programs to provide consultation and guidance with difficult cases, assure communication in multiple forums with counseling staff to be updated on consumer progress and outstanding issues or concerns. You will prescribe psychotropic and/or addiction medication to ameliorate symptoms; review incoming physical exams to ensure if the information is current and accurate. You will communicate to primary counselor the need to make referrals for physical exams with consumer's primary care physician. Ensure that all confidential and potentially sensitive information is processed, maintained, and utilized in a strictly confidential manner. Review and sign all treatment plans for patients enrolled in program as needed. Document all services provided in the case record. Must be literate with electronic health records.

Requirements

  • Master's Degree in a Nurse Practitioner Program and National Board Certification in specialty area required.
  • Current license to practice as a Nurse Practitioner in New York State, required.
  • Current license to practice as a Registered Professional Nurse in New York State, required.

Nice To Haves

  • Specific experience with individuals with mental health needs - preferred.
  • Completing all required collaboration hours by time of hire - preferred.
