About The Position

Horizon is seeking a Nurse Practitioner (NP) to join their Mental Health and Addictions team. The role offers a supportive, team-based work environment with a seamless onboarding experience. As an NP, you will provide initial psychiatric examinations, follow-up medication management, and participate in clinical case conferences. You will prescribe psychotropic and/or addiction medication, review physical exams, communicate referral needs to primary care physicians, ensure confidentiality of sensitive information, review and sign treatment plans, and document all services in electronic health records.

Requirements

• Master's Degree in a Nurse Practitioner Program and National Board Certification in specialty area required.
• Current license to practice as a Nurse Practitioner in New York State, required.
• Current license to practice as a Registered Professional Nurse in New York State, required.

Nice To Haves

• Specific experience with individuals with mental health needs - preferred.
• Completing all required collaboration hours by time of hire - preferred.

Responsibilities

• Provide initial psychiatric examinations for individuals admitted to mental health, rehabilitation and recovery programs and provide follow up medication management services.
• Participate in clinical case conference and case reviews to programs to provide consultation and guidance with difficult cases, assures communication in multiple forums with counseling staff to be updated on consumer progress and outstanding issues or concerns.
• Prescribe psychotropic and/or addiction medication to ameliorate symptoms; reviews incoming physical exams to ensure if the information is current and accurate.
• Communicate to primary counselor the need to make referrals for physical exams with consumer's primary care physician.
• Ensure that all confidential and potentially sensitive information is processed, maintained, and utilized in a strictly confidential manner.
• Review and sign all treatment plans for patients enrolled in program--- as needed.
• Document all services provided in the case record. Must be literate with electronic health records.
