About The Position

We are seeking an experienced Psychiatric Rehabilitation Program (PRP) Direct Service Worker to provide assessment and treatment services that promote recovery, community integration, and improved quality of life for individuals with serious and chronic mental health conditions. Direct Service Workers offer collaborative, person-directed, and individualized services focused on helping individuals develop skills and access resources to reduce maladaptive behaviors stemming from mental health diagnoses.

Requirements

  • At least a high school diploma or equivalent.
  • Previous experience working with emotionally and behaviorally challenged adults or experience working in a recovery-oriented program.
  • Experienced and proficient in using an electronic health record system.
  • Strong interpersonal skills.
  • Ability to travel in the local area and perform field work.
  • Must have a validated driver's license and a dependable vehicle to travel between sites and provide offsite services.
  • Must be actively working toward certification provided by the Commission on Rehabilitation Counseling.

Responsibilities

  • Serve as a liaison between the PRP and Mental Health departments, ensuring collaboration and referral flow.
  • Coordinate and process referrals, ensuring completion of PRP Screening, Letter of Acceptance, and Initial Authorization Request Form.
  • Coordinate the referral process with Revenue Cycle Staff, maintaining consistent communication.
  • Perform monthly chart reviews, document the process, and provide feedback to staff and leadership.
  • Audit record keeping in SAMMS to manage holds and ensure services/forms are completed and signed.
  • Conduct initial assessments to understand client's functional impairments, strengths, abilities, needs, and preferences.
  • Develop individualized, patient-centered treatment plans outlining goals, objectives, and interventions.
  • Implement services through onsite (clinic) and offsite (home/community) activities, including teaching, instruction, encouraging positive change, skill demonstration, promoting wellness and recovery, assisting with self-care and social skills, facilitating independent living strategies, and exposing individuals to academic/vocational/employment resources.
  • Provide services through face-to-face interactions and telehealth.
  • Complete documentation requirements for treatment planning, monthly summaries, progress notes, and other assigned documents.
  • Facilitate monthly collaborations with the client's assigned mental health therapist and treatment team.
  • Maintain clinical records and compile documentation required by CCG and governing agencies, including signing all forms and services.
  • Participate in staff meetings, program clinical team meetings, clinical supervision sessions, and in-service training.
  • Understand and support the CCG philosophy and treatment modalities.
  • Ensure confidentiality regarding client issues, background, and treatment.
  • Maintain case files within the electronic health record (EHR).
  • Perform other duties as assigned.
