The organization is currently seeking an experienced Psychiatric Rehabilitation Program (PRP) Direct Service Worker. Direct Service Workers are responsible for providing assessment and treatment services that promote recovery, community integration, and improved quality of life for individuals with serious and chronic mental health conditions. This role involves delivering collaborative, person-directed, and individualized services focused on helping individuals develop skills and access resources to manage behaviors stemming from mental health diagnoses.

Requirements

  • At least a high school diploma or equivalent.
  • Previous experience working with emotionally and behaviorally challenged adults or experience working in a recovery-oriented program.
  • Must be experienced and proficient in using an electronic health record system.
  • Strong interpersonal skills.
  • Ability to travel in the local area and perform field work.
  • Must have a validated driver’s license and a dependable vehicle to travel between sites and provide offsite services.
  • Must be actively working toward certification provided by the Commission on Rehabilitation Counseling.

Responsibilities

  • Work scheduled shift of Monday - Friday 6am - 2pm or 7am - 3pm
  • Serve as a liaison between the PRP and Mental Health (“MH”) Departments, ensuring consistent collaboration and flow of referrals.
  • Coordinate and process referrals received from MH staff, ensuring completion of the PRP Screening, Letter of Acceptance, and Initial Authorization Request Form.
  • Coordinate the referral process with Revenue Cycle Staff, ensuring consistent communication.
  • Perform monthly chart reviews, document the process, and provide feedback to staff and leadership.
  • Audit record keeping in SAMMS to ensure holds are managed and services/forms are completed and signed.
  • Conduct initial assessments to understand client’s functional impairments, strengths, abilities, needs, and preferences.
  • Develop individualized, patient-centered treatment plans outlining goals, objectives, and interventions.
  • Implement services through onsite (clinic) and offsite (home/community) settings, including teaching, instruction, encouraging positive change, skill demonstration, promoting wellness and recovery, assisting in self-care and social skills restoration, facilitating independent living strategies, and exposing individuals to academic, vocational, and employment resources.
  • Facilitate culturally relevant activities to improve social relationships.
  • Provide services through face-to-face interactions and telehealth.
  • Complete documentation requirements for treatment planning, monthly summaries, progress notes, and other assigned documents.
  • Facilitate monthly collaborations with the client’s assigned mental health therapist and treatment team.
  • Maintain clinical records and compile documentation required by CCG and governing agencies, including signing all forms and services.
  • Participate in staff meetings, program clinical team meetings, clinical supervision sessions, and in-service training.
  • Understand and support the CCG philosophy and treatment modalities.
  • Ensure confidentiality of client information, background, and treatment.
  • Exhibit clear, professional, respectful, and effective communication with families and external contacts.
  • Work independently and implement time management strategies.
  • Exhibit a high degree of ethical conduct, integrity, and confidentiality.
  • Ensure client information is handled per HIPAA standards.
  • Maintain case files within the electronic health record (EHR).
  • Perform other duties as assigned.
