About The Position

The Community Health Worker delivers the program model and will plan and implement culturally sensitive and effective in-home family education services, as directed by service recipients, applicable stakeholders and collaborative partners, while ensuring that services provided are in alignment with the Social Determinants of Health through a trauma informed care lens as defined by the agency.

Requirements

  • High school diploma or GED certificate.
  • Complete an approved Nurturing Curricula and be able to utilize various learning techniques in the successful delivery of the Nurturing Curricula.
  • Maintain certification in at least one approved educational curricula required by agency stakeholder.
  • State of Texas vehicle operator's license and proof of liability insurance.

Nice To Haves

  • Completion of Community Health Worker certificate preferred, or experience in community-based environment.
  • At least two (2) years’ experience in the delivery of service those are resident-driven and work with families to address multiple issues in-home and/or community-based environment preferred.
  • Fluent in English and Spanish (read, write and speak) desirable.
  • Certificate or license, as applicable, by the appropriate State agency as applicable.

Responsibilities

  • Plan and implement culturally sensitive and effective in-home family education services.
  • Ensure services provided are in alignment with the Social Determinants of Health through a trauma informed care lens.
  • Explain issues and advancements in the healthcare and human services industries.
  • Align one's own and the organization's priorities and assess and address community needs in an evidence-based and holistic manner.
  • Demonstrate ethics and sound professional practices, as well as stimulate social accountability and community stewardship.
  • Understand other people as well as to accurately hear and understand the unspoken or partly expressed thoughts, feelings, and concerns of others, including cross-cultural sensitivity through a trauma-informed care lens.
  • Transport program and educational learning materials to various locations to include in-home and community sites.
