About The Position

Liza Medical is opening a new primary care practice in Chelsea, MA, fully embedded within the La Colaborativa Survival Center. This practice offers a novel, community-based model of integrative care for vulnerable populations, providing high-quality care integrated with La Colaborativa’s robust wraparound services that address social determinants of health. The practice aims to build trust, dismantle barriers to healthcare access, deliver excellent outcomes, and offer supportive services. As a Promotora at this flagship practice, you will be an integral part of a physician-led primary care team, focusing on building trusting relationships with patients and their families, advocating for their needs, and balancing clinical care with social realities. You will collaborate with an integrated primary care team to improve healthcare outcomes for patients and the community. A core component of care delivery involves leveraging human-enabled technology with Liza Health to ensure seamless patient-doctor communication, and comfort with technology is required.

Requirements

  • At least 1 year of experience in healthcare as a Community Health Worker, LPN, Medical Assistant, Phlebotomist.
  • You either have the Community Health Worker certificate or are willing to become certified.
  • Strong written and verbal communication skills.
  • Experience working on collaborative and diverse teams.
  • Mission driven and invested in the health and well being of the Chelsea and broader La Colaborativa membership community.
  • Brings a positive, solution oriented attitude to their work.
  • Proficiency in computer technology such as typing, navigating the internet and using multiple software systems simultaneously.
  • Fluency in written and spoken Spanish and English.
  • Able to lift 20-30 lbs.
  • Fully vaccinated against COVID, Flu and relevant vaccines required by OSHA/CDC.
  • Authorized to work in the United States.

  • Personal interest in AI is a plus.
  • Successfully completed the La Colaborativa Community Health Worker Program.

Responsibilities

  • Capture relevant information about the patient’s health and healthcare experience while rooming patients, taking vitals, administering point-of-care testing, and performing standard age- and condition-appropriate screening assessments.
  • Assist with the coordination of post-visit care by scheduling appointments with specialists, coordinating referrals, and sharing information with the patient’s internal and external care team.
  • Collaborate with providers to monitor the health of a panel of patients and determine if they are up to date on preventive measures.
  • Participate in the daily operations of a primary care practice, such as answering incoming phone calls, responding to emails, assisting with front desk inquiries, and ensuring the general upkeep of the clinical space.
  • Assist in providing patient education on chronic disease management and coach patients using an action-planning model based on motivational interviewing techniques.
  • Provide anticipatory preventive guidance to families with children by establishing healing relationships with members and families.
  • Partner with staff of La Colaborativa as well as other service providers to make the navigation to different programs feel like a concierge service.
