About The Position

The Care and Connect Rural Health Transformation Project Hub (RHTP) Project Manager contributes to the Calling of Medical Teams International by leading the planning, launch, scaling and operational coordination of the Care and Connect Hub serving Central Oregon. This role oversees project execution across people, processes, and partnerships to ensure the hub is implemented on time, within budget, and aligned with program goals to improve access to coordinated care and supportive services. The Project Manager ensures responsible use of funds, strong partner engagement, and reliable implementation of workflows that support effective care coordination. Requirements

  • Bachelor’s degree in public administration, health services, social services, business, or a related field (or equivalent experience).
  • 3 to 5 years of project management experience, preferably in healthcare, human services, care coordination, or community based programs.
  • Demonstrated experience managing projects with fixed or startup budgets.
  • Experience working with diverse stakeholders, including community based organizations and public sector partners.
  • Basic knowledge of humanitarian standards, donors, systems and principles (e.g., Sphere, CHS, Red Cross Code of Conduct, and other program standards, etc.).
  • Basic understanding of health systems.
  • Knowledge of equity centered and community informed approaches.
  • Moderately skilled with office software: Microsoft Word, Excel, and PowerPoint.
  • Moderately skilled with communication software: Outlook, Skype, Internet, CRM (or other database programs).
  • Strong organizational, time management, and documentation skills.
  • Skilled at building and maintaining a high functioning, high performing work team.
  • Skilled in strategic and operational thinking.
  • Skilled in fiscal and resource accountability.
  • Ability to work flexible schedule and weekends as needed.
  • Frequent travel for organizational or departmental offsite meetings.

Responsibilities

  • Lead end to end project management for the Central Oregon Care and Connect Hub, from startup through early operations.
  • Develop and maintain detailed project plans, timelines, milestones, risk logs, and implementation checklists.
  • Develop, implement and coordinate a phased hub rollout, ensuring readiness across staffing, workflows, systems, and community partners.
  • Identify project risks and dependencies and implement mitigation strategies to keep deliverables on track.
  • Manage project activities within the approved Care and Connect budget, ensuring fiscal responsibility and transparency.
  • Track project expenditures, resource utilization, and staffing allocation tied to deliverables.
  • Ensure project decisions align with budget constraints while maintaining program quality and compliance.
  • Proactively facilitate the growth of Medical Teams International programs by supporting the development of institutionally funded program proposals and concept notes, as well as supporting financial and program reporting to donors.
  • Collaborate with Financial Control and Performance Analyst and US Programs (USP) Executive Director and Program Managers to forecast needs, monitor variances, support required reporting, share program information, and ensure data is accurate and up to date.
  • Serve as the primary project liaison among internal teams, community based organizations, health partners, and regional stakeholders.
  • Coordinate onboarding, communication, and readiness activities with hub partners across Central Oregon.
  • Facilitate regular project meetings, working groups, and implementation check ins.
  • Support alignment between hub operations and regional service delivery priorities.
  • Collaborate with Care and Connect leadership to design and implement standardized hub workflows, intake processes, communications and management procedures, and escalation protocols.
  • Support integration of data tracking, reporting, and quality improvement processes.
  • Ensure operational and program practices support equity, accessibility, and trauma informed service delivery.
  • Develop and track project metrics tied to timelines, outputs, and early outcomes.
  • Prepare clear, concise project status updates for leadership, funders, and partners.
  • Identify opportunities for efficiency, scalability, and process improvement as the hub matures.
  • Support documentation required for audits, compliance, and funding accountability.
  • Ensure accountability is integrated into strategies, program design, monitoring and evaluations, recruitment, staff onboarding, training and performance management, partnership agreements, and highlighted in reporting.
  • Ensure the Safeguarding of program participants and employees through promoting an environment of awareness and upholding the code of conduct provisions of Medical Teams Protection from Sexual Exploitation, Abuse & Harassment, Child & Vulnerable Adult protection, and harassment free workplace policies.
  • Incumbents will remain alert and responsive to any child and adult safeguarding risks, acquire relevant knowledge and skills which will enable you to promote strong safeguarding practices, understand the child and adult safeguarding policy and procedures, and conduct yourself in a manner consistent with the Safeguarding code of conduct and behavior expectations.
  • Utilize volunteers when appropriate.
