Project Manager, NYREACH

Essen Medical Associates•Bronx, NY

About The Position

NYREACH is a Bronx-based nonprofit focused on addressing health disparities through community-driven solutions. It connects individuals to healthcare and essential social services while building partnerships across New York City. The Project Manager coordinates NYREACH’s concurrent projects, including the Social Care Network (SCN), Medicaid Recertification, Workforce Development, and other core programs. The role maintains the master project plan, clarifies project ownership and next steps, keeps work on schedule, and provides leadership with accurate status information. Projects may involve health plan contracts, state waiver requirements, and fixed deadlines. This is a coordination role; it does not supervise the navigator field team or lead business development. The Project Manager works alongside Managers, the Compliance & Data Analyst, and program staff. The position coordinates SCN workstreams and health plan deliverables; plans Medicaid recertification outreach and program changes; supports Workforce Development and other programs; coordinates new launches and pilots; and works with internal teams and external partners. It also prepares status reports, maintains organized project documentation in SharePoint, and tracks contract, data-sharing, and documentation requirements. Performance targets include weekly status updates for all active projects, at least 90% of milestones and contract deliverables completed on time, timely escalation of risks, and on-schedule weekly reports.
