Project Manager, NYREACH

Essen Medical Associates•New York, NY
•$66,000 - $70,000

About The Position

NYREACH is a Bronx-based nonprofit focused on addressing health disparities through community-driven solutions. It connects individuals to healthcare and essential social services and builds partnerships across New York City. The Project Manager coordinates multiple concurrent projects across NYREACH, including the Social Care Network program, Medicaid Recertification, Workforce Development, and other core programs. The role maintains schedules, ensures projects have clear owners and next steps, and provides leadership with accurate status information. The Project Manager owns the master project plan, conducts regular check-ins with team leads, and follows up on action items through completion. This is a coordination role that works alongside managers, the Compliance & Data Analyst, and program staff; it does not supervise the navigator field team or lead business development. The position coordinates project workstreams, deadlines, workflows, audits, outreach campaigns, program changes, staff onboarding milestones, partner logistics, and program launches. It also prepares status updates and summaries, maintains project documentation, and tracks contract, data-sharing, and documentation requirements. Performance targets include weekly status updates for all active projects, on-time completion of at least 90% of milestones and contract deliverables, and escalation of all risks and delays before deadlines.
