About The Position
Salt Lake Behavioral Health is seeking a Per Diem Program Therapist (Social Worker/(A)CMHC) for our Rehab Units. This position is part of the social services team and will complete biopsychosocial assessments, develop treatment plans, provide individual/group therapy, and assist in discharge planning. The Program Therapist will work collaboratively with the treatment team, families, community resources, and other healthcare providers.
Requirements
- Master's Degree from an accredited college or university, in Social Work or a clinical related mental health field required.
- CSW, LCSW, ACMHC, CMHC, AMFT, LMFT with the state of Utah
- CPR certification is required.
- Successfully complete our current training for restraints and seclusions within 90 days of employment.
Responsibilities
- Prioritize daily tasks including new admissions and discharging patients along with seeing patients on given case load.
- Participate daily in a multi-disciplinary treatment team meeting.
- Conducts psychosocial assessments, ASAM’s and health disparities form for new admissions.
- Educate, communicate and coordinate discharge plans with patient’s families and support systems.
- Provide daily individual therapy along with psychoeducational groups.
- Complete discharge planning including crisis safety plans, family meetings and discharge summaries.
- Complete documentation in a timely manner according to the given standards of the hospital.
