About The Position

The Program Specialist coordinates care for individuals receiving CLHS services and ensures that JEVS Core principles are integrated into the supports and services chosen by each individual. The role coordinates supports across areas such as health care, behavioral services, daytime engagement, employment, communication, skill development, socialization, and recreation, and works with the individual’s support network to adapt program plans as needed.

Responsibilities

• Coordinate the securing of supports and services for individuals, including medical care, therapists, financial resources, behavioral supports, daytime engagement activities, employment, communication, skill acquisition, socialization, and recreation.
• Coordinate specialized staff training specific to each individual’s ISP program plans.
• Participate in the behavioral therapy process and ensure completion of 90-day medication reviews, as appropriate.
• Ensure behavior plans are implemented and data collection is tracked.
• Participate in problem-solving and maintain open communication with the Program Director regarding programming issues.
• Assess each individual’s functioning in relation to their outcomes and social and environmental supports, and adapt program plans as needed.
• Maintain regular contact with people supporting the individual, including supports coordinators, friends, family, neighbors, medical personnel, day programs, and consultants.
• Act as team lead in conjunction with the assigned supports coordinators for each individual on the caseload.
