Nice To Haves

  • Bachelor's degree in Hospital Administration or similar filed.
  • Two (2) years experience in related field.
  • Preference will be given for experience in Behavioral Health field and/or experience in providing nursing care and has experience in quality assurance, performance improvement, and risk management in a healthcare setting.
  • Lean Six Sigma preferred.

Responsibilities

  • Review, code, and classify incident reports based on type, severity, and relevant Quality of Care (QOC) indicators to support accurate data tracking, trend analysis, and regulatory compliance. Ensure consistent application of standardized coding guidelines. Collaborate closely with the QA Specialist on incident report coding and video review processes, and document all incidents requiring video review in the Video Tracker Log daily. Follow-up on pending incident report reviews.
  • Collect and process patient-specific Public Records Requests, including Incident Reports, Complaints, Grievances, Appeals (CGA), and Hospital policies. Track all requests using the Hospital's tracking system and submit them to the Office of Administrative Counsel & Rules (OACR) with appropriate HIPAA-compliant redactions in accordance with Hospital policy and regulatory requirements. Conduct follow-up with OACR every 30 days until requests are fulfilled and keep patient's apprised.
  • Assist the Hospital's Patient Rights Advocate (PRA) in addressing patient concerns and, when appropriate, submitting reports to Adult Protective Services (APS) and the Arizona Department of Health Services (ADHS) related to allegations of abuse, neglect, or exploitation. Support and coordinate on-site APS investigations as needed.
  • Prepare and submit daily Executive Risk Management Team (ERMT) meeting minutes, summarizing incidents and providing them to the Chief Quality Officer (CQO) each morning. Additionally, record and submit monthly minutes for Hospital QAPI committee meetings including Zero Harm, Utilization Management, and Quality Council.
  • Assist the Hospital's Performance Improvement (PI) Specialist with data collection, tracking, and trending, and support performance improvement projects and initiatives.
  • Collect and review Seclusion & Restraint (S&R) packets, ensuring S&R packet supports the submitted incident report. Complete S&R packet audit, in collaboration with medical records, to report compliance during monthly Hospital committees.
  • Prepare and redact required documentation for the Hospital's monthly Independent Oversight Committee (IOC), including Incident Reports and Complaints, Grievances, and Appeals (CGA).
