About The Position

CAN Community Health is looking for a passionate Program Specialist I/Community Health Worker to join their Education & Prevention Team. This entry-level role focuses on serving individuals impacted by HIV, Hepatitis C, STIs, and other infectious diseases. The position involves HIV/STI testing, health education, risk reduction counseling, and linking clients to prevention programs like PrEP. The role also includes community outreach, event planning, distributing educational materials, and maintaining accurate documentation. The ideal candidate will have a basic understanding of HIV prevention and treatment pathways and be committed to patient care and making a difference.

Requirements

• High school diploma required.
• Must be able to operate a motor vehicle and have valid insurance and driver’s license.
• Must be able to pass a Level I and Level II Background check as required.

Nice To Haves

• Minimum 1 year of experience in a related field preferred.
• Relevant college-level coursework may substitute for experience.

Responsibilities

• Provides HIV/STI/Hepatitis C testing and risk reduction counseling in community-based and clinic settings, including phlebotomy.
• Provides education on Pre-exposure Prophylaxis (PrEP) to clients and community members and links clients to the PrEP program.
• Ensures client confidentiality 100% of the time by conforming to HIPAA laws and CAN Community Health Policies and Procedures.
• Accurately documents the provision of testing in the electronic medical record, state reporting systems, and on paper as required.
• Fosters and assists in developing new community partnerships through various agreements under the supervision of the HIV Prevention Program Manager and Prevention Team Lead.
• Serves as a representative of CAN Community Health and conducts community-based outreach in various settings.
• Assists in the planning and execution of events including testing, tabling, and provides health education.
• Distributes condoms and other promotional materials to community members and partner organizations.
• Accurately documents and maintains all records related to prevention data collection efforts.
• Represents CAN Community Health at local, state, and national conferences and meetings, as deemed necessary.
• Collaborates with other disciplines internally and externally to coordinate client/patient services and community needs.
