About The Position

The Supported Living Program Manager oversees Direct Support Professionals, Lead DSPs, and, when applicable, Assistant Program Managers. The Program Manager is responsible for the daily operation of Supported Living services and works with ISP Teams and supported individuals to ensure quality services and supports that promote independence and community inclusion.

Ability to pass an initial criminal history check from the Department of Human Services Criminal Records Unit and recheck every 2 years after.

Must be at least eighteen years old and hold a current driving license, insurance, and a good driving record.
Ability to complete required training and annual training thereafter.

One year of experience working with individuals with Intellectual and Developmental Disabilities.
Intermediate computer/technical literacy.

Nice To Haves

Associate’s or Bachelor’s degree in Human Development, Psychology, or related field.
Knowledge of the Oregon Developmental Disabilities Services and Adult I/DD Case Management systems.
At least 6 months of management or supervisory-related experience.
Experience using online systems, such as “When to Work”, eXPRS, and Therap.
Knowledge of I/DD Individual Support Plan forms and implementation.
Ability to perform advanced computer operating skills.
Training and experience in Dual Diagnosis (I/DD and Mental Health).

Responsibilities

Oversee Direct Support Professionals, Lead DSPs, and Assistant Program Managers.
Responsible for the daily operation of Supported Living services.
Work with ISP Teams and supported individuals to ensure quality services and supports that promote independence and community inclusion.
Organize and facilitate staff meetings.
Evaluate and document the performance of Support Staff routinely.
Follow up with disciplinary training in the event that a staff person is not performing their job according to acceptable criteria.
Give timely positive feedback verbally or in writing in the event that a staff person is meeting or exceeding criteria.
Submit all written feedback to the Services Coordinator.
Assist in reviewing and approving time sheets and mileage forms.
Carry the on-call emergency phone when scheduled and respond to calls as needed.
Attend all applicable agency meetings (staff, management, etc.).
Assume responsibilities of the Services Coordinator in their absence (as needed).
Ensure that the individuals’ quality of life is in keeping with Home Life philosophy and values, policies, and procedures by assisting in coordinating, training, and enforcing staff support.
Directly supervise a group of staff as assigned and manage a set caseload of supported individuals.
Address staff concerns, problems, and grievances and respond in a positive, proactive way.
Adhere to all policies, rules, and procedures of Home Life, Oregon Developmental Disability Services, DHS, and Oregon Administrative Rules (OAR).
Establish and maintain effective communication regarding the supported living program with parents/guardians and supported individuals.
Inform the ISP Team as appropriate, regarding medical, behavioral, personal status changes, and concerns in a timely manner, while maintaining confidentiality.
Delegate responsibilities/work assignments among staff to ensure that all required tasks are completed.
Ensure that staff are following their scheduled shifts and accurately recording hours.
Assist in coordinating staff schedules, including substitutes, interns, and volunteers, to ensure adequate coverage of the program.
Coordinate, adjust, and publish a weekly schedule for all staff schedules, including substitutes, interns, and volunteers, to meet the supported individual care needs and communicate schedule changes to staff in a timely manner, and hold staff accountable for all scheduled shifts with the assistance of the Program Managers.
Approve requests for time off in accordance with the program as well as agency needs.
Meet frequently with the Program Managers to plan scheduling.
Review time sheets, in-service records, personal mileage forms, and complete monthly payroll.
Ensure that all documentation within the program is maintained at levels that meet required standards and policies.
Follow the Individual Support Plan(s) and protocols of supported individuals as outlined by the ISP team.
Complete General Event Reports (GER) and Therap progress notes (Tlogs) in accordance with directives and training in a timely manner and consistent with the OAR guidelines.
Provide an administrative follow-up in all assigned GERs that includes the prevention and retraining of staff as needed to prevent future incidents.
Ensure that GERs are reported to the appropriate people within the incident reporting timelines.
Ensure that the supported individuals’ medical needs are met according to their ISP and support needs.
Respond appropriately to the posted Emergency Preparedness Plan and/or on-call procedures.
Document and update supported individuals’ medical records, e.g., Medical updates, medical orders, immunizations, weights, ongoing medical records, and MARs if used by the individual, and ensure that this is also being done by support staff.
Make and attend appointments and record information accurately and completely.
If applicable, become delegated to perform medical tasks by a licensed, contracted Registered Nurse and ensure that staff are adequately trained.
Ensure that medications and health supplies are maintained at an adequate supply level as applicable.
Schedule and accompany clients to medical appointments (or delegates to DSP when appropriate) and complete all corresponding paperwork.
Review the Medication Administration Record weekly.
Update the Medication Administration Records, medication protocols, and complete 180-day reviews as needed.
Notify the ISP team if new supports are necessary in maintaining individuals’ health and safety.
Follow Mandatory Abuse Reporting standards and guidelines.
Manage the petty cash and financial benefits for supported individuals, when applicable.
Accesses program’s petty cash to provide to staff for program expenses.
Performs the duties of a Representative Payee (on behalf of Home Life), including reporting wages to SS, maintaining annual paperwork, budgeting, and making sound financial decisions to protect their finances and limit risk of financial exploitation.
Ensure the safeguarding of finances and that individual funds are maintained in a safe, secure space.
Read, ensure understanding, and write supporting documentation to continually follow supported individuals’ ISP and all other support plans as applicable (nursing or positive behavior support).
Use the least intrusive supportive services to assist and encourage supported individuals towards the goals within their ISPs to live as independently as possible within a safe, healthy environment that fosters growth with the understanding that they have many capabilities, are valuable, and contribute to the community.
Ensure that Direct Support Professionals and supported individuals’ routines are being followed, as agreed upon (e.g., work routine, day supports, appointments, and all other activities).
Advocate for supported individuals and attend assigned meetings and appointments.
Assist DSPs in supporting individuals to explore options and safeguard their personal choices.
Assist DSPs in supporting individuals in understanding their rights, responsibilities, and freedoms to facilitate individual independence.
Assist DSPs to support individuals in developing and maintaining positive connections in the community; model and teach safety and appropriate social behavioral etiquette.
Works with DSPs to support individuals in maintaining and growing relationships with friends and families.
Assume additional responsibilities as assigned by the Services Coordinator or upper management.
