About The Position

Mains’l Services is seeking a compassionate and experienced Program Manager to lead a 245D program in Big Lake, Minnesota. The role supports people with disabilities in living meaningful, self-directed lives in their homes and communities. The Program Manager leads Direct Support Professionals (DSPs), builds relationships with individuals receiving services, and collaborates with families and stakeholders to ensure high-quality care. The position oversees person-centered support in hourly and 24-hour supported living services, team development, regulatory compliance, and resource and budget management.

Requirements

• Meet one of the following Minnesota 245D qualification pathways: a bachelor’s degree in human services, education, or health and one year of direct care experience supporting people with disabilities or individuals age 65 and older; an associate degree in a related field and two years of such direct care experience; a diploma from an accredited institution in a related field and three years of such direct care experience; or 50 hours of relevant training and four years of such direct care experience.
• Be at least 18 years of age.
• Have a valid Minnesota driver’s license and reliable transportation with insurance.
• Be able to pass a DHS background check.

Responsibilities

• Lead and support DSPs delivering person-centered care in hourly and 24-hour supported living services.
• Develop individualized annual support plans that reflect each person’s goals, preferences, and strengths.
• Train, coach, schedule, and develop the team to support high-quality service delivery.
• Communicate and collaborate with case managers, guardians, and other professionals.
• Ensure compliance with licensing, health, and safety standards.
• Manage resources responsibly and meet budgetary goals.
