Requirements

Experience working in residential services, disability services, or a related human services field
Knowledge of Minnesota 245D licensing requirements and DHS regulations
Designated Coordinator qualifications as defined by Minnesota Statute
Strong person-centered planning and individualized service plan (ISP) development skills
Excellent communication, organizational, and documentation skills
Ability to pass a background study through the Minnesota Department of Human Services
Compassionate, dependable, and committed to advocating for individuals with disabilities

Responsibilities

Develop, implement, and monitor individualized service plans (ISPs) for individuals receiving residential support services
Serve as the primary point of contact for individuals, families, case managers, and other stakeholders
Coordinate and facilitate person-centered planning meetings and reviews
Ensure compliance with DHS licensing standards, 245D regulations, and all applicable state and federal guidelines
Supervise and support direct support professionals (DSPs) to ensure quality care delivery
Maintain accurate documentation, progress notes, and service records
Conduct assessments and identify support needs, goals, and outcomes for each individual served
