About The Position

This position is responsible for the management of the Housing and Health Opportunities (H2O) program. This position is also responsible for the care coordination of eligible members in securing safe, affordable, and sustainable housing through individualized housing support, coordination with AHCCCS & Solari, and collaboration with community resources. The Program Manager will ensure quality Pre-tenancy and Tenancy Sustaining services (PTTS) through the agency’s H20 program. The position guides our Program Supervisors and ensures our Housing Navigation (HN) team provides case management to members by using best practices and treating members in a dignified and sensitive manner. Management responsibilities will include the supervision of 5-7 staff. Under the general supervision of the Regional Director, this position is responsible for assisting members in meeting their expressed goals while living successfully in the community. This position requires some travel to the various cities of Maricopa County.

Requirements

  • Master of Social Work is a significant value and/or bachelor’s degree in social work or related work field.
  • Minimum of 3-5 years of experience providing housing services to homeless individuals and/or individuals with mental health or substance abuse issues preferred
  • Minimum of 3-5 years of experience in program management and staff supervision preferred
  • Minimum of 3-5 years of experience in care coordination and case management for the following populations: Individuals/ Families experiencing homelessness, Individuals at risk for avoidable hospital stay (High Utilizers), Individuals w/Serious Mental Health and or SUD, Adult Nursing Facility Residents Transitioning to the Community, Adults w/intellectual or developmental disabilities preferred
  • A valid AZ driver’s license, proof of insurance, and a reliable vehicle are required.
  • Must be comfortable utilizing your own vehicle to conduct home visits, visit clients at shelters, meet clients out in the community, etc.
  • Ability to carry 3-5 clients at any given time
  • Working knowledge of a strengths based, person-centered approach
  • Strong interpersonal, organizational, written and communication skills
  • Ability to multitask, meet deadlines and maintain member privacy, including protecting the confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities
  • Sensitivity to service population’s cultural and socioeconomic characteristics
  • Excellent verbal and written communication skills
  • Ability to maintain professional conduct, attitude, and appearance at all times
  • Ability to work collaboratively with others and a willingness to participate fully in the team process
  • Proficient in Microsoft Office programs (e.g., Word, Excel, PowerPoint)
  • Requires ability to sit up to 3-6 hours per day with intermittent occasional walking and standing

Nice To Haves

  • Fluency in Spanish is a plus
  • Experience with HMIS Data Entry strongly preferred

Responsibilities

  • Manage the day-to-day operations of the H2O program
  • In collaboration with the Regional Director hire, train, supervise, evaluate, and terminate program staff as necessary
  • Conduct 90-day, and annual reviews for Supervisors/ Program administrators
  • Manage client flow and HN/PTTS caseloads in collaboration with the Program Supervisors
  • In collaboration with the Housing Solutions Supervisors review and keep track of monthly invoices
  • Attend meetings assigned by the Regional Director and be prepared to present on service delivery, caseloads, and problem-solving program challenges
  • Guide staff in providing case management utilizing trauma informed care and strengths-based approach, which are in line with Housing First and harm reduction principles
  • Provide intervention for program participants and serves as a point of contact for program grievances
  • Ensure the team understand and provide PTTS Core Components: outreach and engagement, comprehensive assessment, and care management place, enhanced care coordination, health promotion, comprehensive transitional care, member and family support, coordination of and referral to community and social support services
  • Keep up with the after-hour cellphone that is assigned to program staff and provide on call coverage as needed
  • Supervise Housing Solutions Supervisors / Program Administrators
  • Demonstrate leadership and team building qualities to implement the mission and core values of the agency
  • Work with staff, and MSW interns to implement the mission and ensure quality service delivery
  • Oversee quality control of case files on a regular basis to ensure accurate, up to date documentation on all clients, including but not limited to Individual Service Plans, Emergency Plans and Health Needs Assessment, care plans and case notes, etc.
  • Facilitate staff meetings and case conference meetings in collaboration with Regional Director
  • Meet with Supervisors bi-monthly to discuss program challenges/needs
  • Provide leadership to staff in emergency situations
  • Attends internal and external meetings on behalf of the agency and housing solutions team
  • Must participate in networking functions, community meetings, meet with funders and partnering agency
  • Assist with program activities when necessary
  • Attend staff meetings and training workshops as needed
  • Enhance job performance by applying up-to-date professional knowledge gained by attending seminars and conferences and reviewing professional publications
  • Develop new partnerships with other agencies to enhance our current services
  • Perform ad hoc projects as appointed by Housing Solutions Director
