About The Position

This role is responsible for providing clinical Social Work coverage on the Neurodegenerative Disease (ND) unit. The Program Manager ensures that resident assessments and comprehensive care plans reflect the specific needs of each resident. They are the primary point of contact for resident and family complaints, ensuring timely resolution and reporting interventions to the Administrator. The position involves comprehensive case management, including admission and discharge assessments, MDS assessments, care plan development, and advocacy for residents. This includes coordinating services with internal departments and external agencies, interpreting resident needs to staff and families, and providing counseling regarding disease progression. The role also includes active participation in Quality Improvement initiatives, conducting audits, and contributing to the QAPI meeting. Additionally, the Program Manager oversees the admission process, ensuring potential residents are appropriate for the unit, manages inquiries, conducts pre-admission reviews, maintains waiting lists, and coordinates admissions with other units. They work closely with the Nursing Department to ensure maximum resident scoring during MDS reviews and follow up on documentation. Environmental rounding to ensure a homelike environment and reporting needed repairs are also key responsibilities. The Program Manager provides psychosocial counseling, participates in unit planning, collaborates with Staff Development for education, and coordinates resident activities with Therapeutic Recreation and Nursing. They also coordinate educational programs related to neurodegenerative disease and are responsible for ensuring all Social Work documentation is complete and timely according to state guidelines. Participation in the Ethics Committee and QA/QI committee, and assistance with budget preparation are also expected.

Responsibilities

Provide clinical Social Work coverage on the ND unit.
Ensure resident assessment and comprehensive care plan reflect specific resident needs.
Respond to all resident and family complaints, ensuring timely resolution.
Report interventions and action plans to the Administrator.
Provide comprehensive case management to all residents on the ND unit.
Conduct admission and discharge assessments and screening.
Ensure timely completion of MDS assessment and comprehensive care plan.
Advocate through communication with all disciplines on behalf of the residents.
Coordinate needed services with the Center and with outside agencies.
Interpret resident needs and behaviors to staff and designated representative.
Maintain regular communication with designated representatives regarding resident specific issues.
Provide counseling to residents and families regarding disease progression.
Actively participate in all Quality Improvement initiatives on the unit.
Complete needed audits and reports at the monthly QAPI meeting.
Assure the admission process is thorough and potential residents are appropriate for the unit.
Coordinate the administrative function of admissions to the unit.
Receive all inquiries regarding potential residents and respond.
Conduct pre-admission committee decision-making on potential residents.
Maintain on-going waiting list.
Coordinate admission to other SNF units if beds are available.
Maintain dialogue with discharging institutions for on-going referrals.
Work closely with Nursing Department assessors during MDS reviews to ensure maximum scoring of residents on unit.
Conduct follow-up with departments if documentation in the medical record is lacking or missing.
Ensure that all required information is present at review.
Conduct environmental rounding on the unit to ensure a homelike environment and report needed repairs to maintenance.
Provide psychosocial counseling to residents, families and friends.
Coordinate the Family Council and support group meetings.
Participate in the short and long-term planning process for the ND Unit with Administration.
Collaborate with Staff Development Department to provide initial and continued education to care members regarding ND.
Participate with Therapeutic Recreation and Nursing in coordination for resident activities.
Coordinate all educational programs relating to neurodegenerative disease with appropriate departments and outside sponsors.
Report to work at various times, including weekends, evenings and nights.
Report administrative issues that arise on the ND unit.
Ensure that all required Social Work assessments, care plans, progress notes, MDS, and documentation is complete according to quality and timeliness.
Ensure the completion of all patient Level 1 and Level 2 SCREENS according to New York State Department of Health guidelines, and PRls for ND unit.
Active Participant on the Ethics Committee and QA/QI committee, if required.
Assist with budget preparation for unit.
