Wellness Equity Alliance is a national multidisciplinary health organization that designs and delivers integrated, community-based care for populations most impacted by health inequities. WEA has launched a unique street medicine program featuring mental health, substance use disorder treatment, and infectious disease! The most vulnerable people experiencing homelessness have likely been failed by institutions many times in their lives, and their mistrust of authorities, institutions, and individual care providers may represent an attempt at self- protection that, over time, becomes a barrier to accessing care and resources that could improve their lives. The street medicine team will work to build trusting relationships with people who are in need of medical services, work to decrease the logistical barriers that block access to health care and provide care directly to the places where unhoused individuals live. We are seeking an experienced Program Manager to join our team. The successful candidate will be responsible for overseeing the development and implementation of care management programs that improve patient outcomes and reduce healthcare costs. The CalAIM Program Manager will work closely with healthcare providers, patients, and their families to ensure that patients receive the highest quality of care by ensuring compliance with state regulations and alignment with our mission, providing training and ongoing support to teams, and overseeing program development and evaluation and reporting.
