About The Position

Wellness Equity Alliance is a national multidisciplinary health organization that designs and delivers integrated, community-based care for populations most impacted by health inequities. We do this through mobile and field-based models, providing medical care, behavioral health services, substance use treatment, harm reduction, and care coordination in nontraditional settings such as encampments, schools, reentry sites, and rural communities as well as with sovereign tribal nations. WEA has launched a unique street medicine program featuring mental health, substance use disorder treatment, and infectious disease! The most vulnerable people experiencing homelessness have likely been failed by institutions many times in their lives, and their mistrust of authorities, institutions, and individual care providers may represent an attempt at self- protection that, over time, becomes a barrier to accessing care and resources that could improve their lives. The street medicine team will work to build trusting relationships with people who are in need of medical services, work to decrease the logistical barriers that block access to health care and provide care directly to the places where unhoused individuals live. We are seeking an experienced Program Manager to join our team. The successful candidate will be responsible for overseeing the development and implementation of care management programs that improve patient outcomes and reduce healthcare costs. The CalAIM Program Manager will work closely with healthcare providers, patients, and their families to ensure that patients receive the highest quality of care by ensuring compliance with state regulations and alignment with our mission, providing training and ongoing support to teams, and overseeing program development and evaluation and reporting. Requirements

  • Bachelor’s degree in social work, public health, healthcare administration, or related field’ Master’s degree preferred.
  • Minimum of 3 years of experience in care management, with at least 1 year in a supervisory/management or program coordination/management role.
  • Experience may be substituted for educational requirements on a year-for-year basis.

Responsibilities

  • Implement and manage the CalAIM program in accordance with the CALAIM Enhanced Care Management and Community Supports Policy Guides, ensuring all services meet state requirements.
  • Supervise and train staff, including care coordinators and support personnel, to deliver high-quality, comprehensive care management services in accordance with all CalAIM programs.
  • Collaborate with healthcare providers, social services, and other community partners to facilitate integrated care and support for CALAIM clients.
  • Monitor care plans that are person-centered, addressing the unique needs and preferences of each client.
  • Ensure the programs meet all documentation and reporting requirements set by the state and federal agencies.
  • Analyze program data to evaluate effectiveness, identify areas for improvement, and implement evidence-based strategies to enhance service delivery.
  • Manage care management documentation system including data quality metrics and reporting.
  • Participate in stakeholder meetings and serve as the primary liaison between WEA and local/state agencies regarding CalAIM program matters.
  • Manage the program budget, including tracking expenditures and optimizing resource allocation for maximum impact.
