About The Position

East Bay Community Action Program (EBCAP) is seeking a dynamic, mission-driven leader for their Assertive Community Treatment (ACT) Team Manager position, focusing on young adults with serious mental illness and co-occurring disorders. This role involves leading a multidisciplinary team in delivering intensive, community-based behavioral health services. The ACT Team Manager will drive measurable outcomes, build strong community partnerships, develop clinical excellence, and ensure quality and compliance with regulatory standards. This is a community-based, full-time role requiring travel, on-call responsibilities, and carrying a reduced clinical caseload.

Requirements

  • Master's Degree in human services, OR Bachelor's Degree in human services with a LCDP or RN license.
  • 3-5 years post-graduate clinical experience in rehabilitative community-based mental health.
  • 1-2 years of managerial and supervisory experience.
  • 2 or more years of experience working with adolescents and young adults in a community setting.
  • Experience serving individuals with Severe and Persistent Mental Illness.
  • Knowledge of co-occurring disorders and Medication-Assisted Treatment (MAT).
  • Familiarity with BHDDH regulations.

Nice To Haves

  • Background in IHH, ACT, CSP, or similar intensive community-based models.
  • Knowledge of ACT fidelity standards and state regulations.

Responsibilities

  • Lead comprehensive care coordination that integrates physical health, behavioral health, and social determinants.
  • Reduce hospitalizations and emergency interventions through proactive, evidence-based treatment planning.
  • Improve client stability, housing retention, and community integration through coordinated support.
  • Collaborate with hospitals, courts, law enforcement, primary care providers, and community agencies.
  • Create seamless care transitions that keep clients connected to the services they need.
  • Strengthen EBCAP's reputation as a leader in community-based mental health care.
  • Mentor and coach a multidisciplinary team through ongoing supervision and competency development.
  • Implement evidence-informed practices that reflect the latest in ACT model fidelity.
  • Foster a culture of continuous improvement, accountability, and compassionate care delivery.
  • Maintain Joint Commission standards and state regulatory compliance.
  • Monitor performance metrics that demonstrate program effectiveness.
  • Lead quality improvement initiatives that enhance service delivery and client outcomes.
