About The Position

The Program Implementer delivers community outreach, overdose prevention education, opioid overdose reversal medication distribution, and peer navigation for YPR’s regional overdose prevention initiative. Working directly with adolescents and young adults ages 14–26, families, and community partners, the Program Implementer helps people recognize and respond to overdose, access prevention resources, and connect with substance use disorder treatment and recovery support. The position provides individualized assistance, warm handoffs, and follow-up to help participants access services.

Requirements

  • Experience in peer support, public health outreach, or social services
  • Familiarity with substance use disorder, overdose prevention, or recovery support strongly preferred
  • Positive and supportive attitude toward all pathways of recovery
  • Sensitivity and awareness of the unique needs and stigmas associated with underserved populations
  • Technology/computer/virtual meeting experience required
  • Excellent organizational, communication, and leadership skills

Responsibilities

  • Respond to work-related communications from YPR employees within a reasonable timeframe during scheduled working hours, generally within one (1) business day, unless alternative arrangements have been approved in advance by the supervisor.
  • Celebrate both large and small victories while identifying and addressing bottlenecks or barriers the team may be experiencing.
  • Comply with all YPR reporting procedures, including ADP time tracking, Concur expense reporting, Salesforce/Community updates, and census reports.
  • Obtain and maintain all required training and certifications as directed by the organization or supervisor, including but not limited to Diversity, Equity & Inclusion (DEI) and Mental Health First Aid (MHFA), Sexual Abuse, and Cyber Security.
  • Represent the organization professionally in all interactions.
  • Handle sensitive and confidential matters with professionalism and discretion.
  • Supports special projects and organizational initiatives.
  • Conduct community outreach. Support and carry out outreach events across the four-county region in collaboration with schools, healthcare organizations, faith communities, recovery organizations, and other partners.
  • Provide overdose prevention education. Deliver education on overdose recognition, response, counterfeit pill risks, and administration of opioid overdose reversal medications using standardized project materials and SAMHSA’s Overdose Prevention and Response Toolkit.
  • Distribute overdose reversal medications. Assist with kit assembly and distribution at outreach events and partner locations, following project procedures and documenting distribution activities.
  • Deliver individualized peer navigation. Help participants identify recovery goals, understand available services, and connect with substance use disorder treatment, mental health services, and recovery support.
  • Facilitate warm handoffs. Actively connect participants with receiving providers and partner organizations, helping them navigate referral steps and barriers to accessing care.
  • Provide participant follow-up. Maintain contact after referrals to assess progress, support engagement, and document whether participants successfully connect with services.
  • Maintain timely service records. Enter participant contacts, outreach activities, education, medication distribution, referrals, warm handoffs, and follow-up outcomes into Pathwell, YPR’s electronic performance management platform.
  • Support continuous improvement. Share participant feedback, unmet needs, and service barriers with the project team to improve outreach and referral practices.
  • Complete required training. Participate in project training on overdose prevention, overdose reversal medication administration, counterfeit pill awareness, and trauma-informed service delivery.
  • Team Collaboration. Work closely with the Program Implementer/Bilingual Specialist, Community Education Specialist, Community Outreach & Partnerships Manager, and Program Coordinator/Data Lead to coordinate outreach, deliver consistent education, connect participants to services, and maintain accurate records.
  • Contribute to the project’s annual targets alongside other staff and community partners.
  • Collaborate to support and implement YPR’s strategic plan while promoting and encouraging the many pathways of recovery.
  • Actively champion diversity, equity, inclusion, and accessibility (DEIA) initiatives.
  • Continuously strive to improve personal performance and contribute to organizational excellence.
  • Use data to inform decision-making, enhance outcomes, and guide organizational development.
  • Perform other related duties and marginal functions as assigned.
