About The Position

The Program Floater provides a wide range of responsibilities within the program structure to support any shortages of staffing resources with flexibility. This role involves direct personal care services, facilitating groups, medication pass, transportation, referral review of client placement, discharge planning supports, and collaborating with the interdisciplinary team to ensure the delivery of exceptional care and services. The Program Floater includes both clerical duties and personal care duties for clients to support client care services as individually needed.

Requirements

  • High school diploma or equivalent
  • Basic First Aid & CPR certification
  • Clear California and Federal background checks
  • A Clear criminal record review and fingerprint background check
  • Successful completion of a Negative Drug Test
  • Hiring is contingent upon a full background screening in accordance with California licensing regulations; DOJ/FBI clearance, pre-employment physical and TB clearance.
  • DMV printout required prior to employment.
  • Basic First Aid certification required prior to employment.
  • Must have an active Driver's License
  • Must provide proof of vehicle insurance coverage of a minimum of $30,000 liability

Responsibilities

  • Provides direct personal care services.
  • Facilitates groups.
  • Administers medication pass.
  • Provides transportation for clients.
  • Reviews client placement referrals.
  • Supports discharge planning.
  • Collaborates with the interdisciplinary team.
  • Contributes to community relationships by demonstrating cooperation and professional conduct with clients, families, colleagues, providers, pharmacies, and other vendors or persons.
  • Assists clients in daily living activities and teaches clients in self-care practices.
  • Advocates therapeutic activities, such as physical exercises, arts and crafts, and recreational games.
  • Facilitates groups as needed by the clinical team.
  • Observes and documents clients' behaviors to facilitate assessment and development of treatment goals.
  • Attends to routine health care needs of clients under supervision of medical personnel and reports client status change to Leadership as needed.
  • Assists clients in taking medication as prescribed.
  • Acts as a liaison between provider, insurance benefit and client as needed.
  • May support the intake referral process to support client placement in programming.
  • Follows up and communicates between client and an array of aftercare resources to ensure a smooth transition of discharge.
  • Records and reports changes in client's condition to leadership and documents with appropriate charting criteria and procedures to record pertinent information in client chart.
  • Provides transportation for clients as needed.
  • Accompanies clients on off-ground trips supporting clients treatment plans as approved by the interdisciplinary team.
  • Observes and makes rounds every hour or as needed to account for all client's presence and safety.
  • Provides breaks and lunches for staff as scheduled or as needed.
  • Provides scheduling support for staff as needed.
  • Maintains a clean, safe, and orderly environment by performing general housekeeping duties to promote a healthy living environment.
  • Adheres to facility policy and procedures, State & Federal law, and Community Care Licensing regulations and standards.
  • Completes client charting on electronic healthcare software (PointClickCare).
  • Provides facility on-call support as needed.
  • Performs other duties as assigned.
