About The Position

This position serves as the Director for the Winnebago Public Health Department Wa i pi nagu (Diabetes) Program. Work involves direct management, fiscal, and personnel oversight of the Diabetes Program and other related grant programs.

Requirements

  • This position is considered administrative/supervisory.
  • Incumbent must possess the ability to work independently and exercise a large degree of leadership, initiative, judgment and professionalism in planning, designing, and implementing diabetes treatment and prevention programs.
  • Incumbent must have the ability to work with people of diverse backgrounds and the experience and education conducive to creating a positive environment.
  • Professional knowledge of basic and advanced management skills, principles, practices, and procedures required to plan, assess, evaluate and implement program goals and objectives.
  • Ability to develop program budgets and to provide monitoring and continuous evaluation of the fiscal management of program accounts and expenditures in accordance with standards and regulations required.
  • Must feel comfortable analyzing data to drive program decisions and direction.
  • Supervisory experience is necessary. Incumbent will be responsible for orientation, supervision, evaluation, and monitoring of professional growth of all employees.
  • Must possess the ability to communicate effectively, orally, and in writing.
  • Must possess the ability to read, write, and speak the English language fluently.
  • Must be able to continually and effectively employ professional verbal and written communications skills.

Nice To Haves

  • Prefer a four-year degree in management, health services, nursing, diabetics, nutrition and/or related science.
  • Number of years of experience in management/supervision/fiscal oversight will be considered in lieu of a four-year degree.
  • Experience in writing federal grants & federal grant management preferred.

Responsibilities

  • Provides professional leadership to plan, implement and evaluate the treatment, management and prevention elements of the Wa i pi nagu Program.
  • Provides direct management and supervision of the Winnebago Public Health Department Diabetes Program personnel (10+). Oversight of specialty consultant contracts such as dietitians and other positions as needed.
  • Plans, organizes and utilizes the resources available to accomplish the goals and objectives of the Wa i pi nagu Program.
  • Responsible for grant management and reporting requirements of the Wa i pi nagu program grants.
  • Provides professional guidance, support and encouragement for staff, to further professional growth and development, to ensure they keep abreast of the latest standards of care, guidelines and best practices within their scope.
  • Responsible for budget preparation, control and modification of program and grant budgets in accordance with specific guidelines, procedures, and regulations.
  • Meets regularly with staff to ensure efficient operation and implementation of program goals and objectives, and to approve program changes or additions.
  • Acts as a liaison between the Wa i pi nagu Program and the Twelve Clans Unity Hospital Medical Team to exchange information, identify program needs and to maximize the delivery of care for the patients, in-home, clinic, hospital, and community settings.
  • Works under the immediate supervision of the Chief Public Health Officer who provides general guidance and support for implementing overall priorities, personnel policies and procedures and the goal and objectives of the Wa i pi nagu Program.
  • Supervise and help coordinate billing procedures for diabetes treatment, management and prevention services to collect reimbursement and promote program sustainability.
  • Responsible for maintaining the diabetes and prediabetes registry and submitting the annual diabetes audit.
  • Responsible for leading collaborative efforts within the community to enhance programming activities, goals, outcomes, and partnerships.
