Requirements

  • Graduate of a program in his/her professional discipline.
  • Hold an applicable and current license to practice.
  • Computer and current technology competence.

Nice To Haves

  • Experience working with an adult and geriatric population is preferred.
  • Experience serving people with physical disabilities is preferred.

Responsibilities

  • Providing therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the discipline's qualifications, professional practices and ethical standards.
  • Contributing to program development, quality improvement, problem solving, and productivity enhancement in a flexible interdisciplinary fashion.
  • Carrying out all responsibilities in accordance with Kindred Healthcare CORE values.
  • Coordinating the development of team members through structured activities.
  • Planning and conducting orientations in conjunction with department supervisors.
  • Assisting Program Director in identifying educational needs of team and implementing corresponding in-service sessions.
  • Attending/presenting at unit in-service sessions as scheduled.
  • Attending external professional growth activities as scheduled and effectively communicating information to the team.
  • Encouraging and assisting with the development of student programs.
  • Assisting the Program Director in providing team leadership.
  • Maintaining open lines of communication between the team and the Program Director.
  • Assisting Program Director with recruitment and interviewing process.
  • Assisting Program Director in team member’s supervision and providing input into team member evaluations as requested.
  • Communicating and demonstrating a professional image/attitude for patients, families, clients, coworkers and others.
  • Adhering to applicable dress code.
  • Adjusting personal schedule to meet needs of unit when requested.
  • Protecting patient confidentiality at all times and facilitating effective communication with all patients.
  • Communicating professionally at all times.
  • Demonstrating and promoting honesty and integrity at all times.
  • Promoting adherence to policies and practices of applicable professional organizations, client hospital.
  • Acting as a resource for team members regarding the contents of applicable professional code, company standard operating procedure/ handbook, unit policy and procedure and hospital policies.
  • Contributing to client relations and marketing activities.
  • Assisting the Program Director in establishing contact with discharge planners, physicians, and other referral sources.
  • Establishing a positive relationship with facility personnel including administration, nursing, and therapy team members.
  • Monitoring team members competency, licenses/certifications, and memberships in applicable professional organizations.
  • Coordinating treatment activity between disciplines and team members.
  • Facilitating clinical staffing and encouraging team members to provide/seek input to/from others on patient progress and planning.
  • Soliciting input from team members on issues relating to patient care, team development, and program development.
  • Maximizing team productivity through effective scheduling of team members/treatment in conjunction with department supervisory team members.
  • Taking immediate action through appropriate channels to correct adverse situations involving team members.
  • Using and updating unit communication methods in a timely manner.
  • Participating in team business meetings.
  • Offering ideas/solutions in problem solving situations/activities regarding the program.
  • Participating in coordination of facility/unit quality management and program evaluation activities.
  • Attending/leading scheduled facility/unit quality management meetings as assigned.
  • Assisting team members in interpretation of reports/data to improve quality of care delivered.
