The Center for Health Equity & Community Wellness (CHECW) aims to eliminate racial and other inequities that lead to premature mortality. CHECW operates with a deep understanding of history and structural analysis, highlighting the harm caused by centuries of racist and socially unjust policies, while working towards redress for the most impacted NYC communities. CHECW addresses inequities across community and healthcare systems in partnership with community, faith-based, and healthcare organizations. The focus is on social determinants of health, including environmental and commercial factors, addressing both upstream and downstream causes to improve the health and well-being of New Yorkers. The Bureau of Harlem Neighborhood Health (Harlem BNH) supports the agency's mission to protect and promote the health of all New Yorkers. Harlem BNH's service area encompasses all of Harlem (East, Central, and West), as well as Washington Heights and Inwood. Harlem BNH concentrates on the structural and root causes of health disparities and develops programs to address these outcomes with input from partners and residents. Addressing the social determinants of health (SDOH) is a key focus, integrated into programs like the Harlem Health Advocacy Partnership and the Asthma Counselor Program. The Bureau also offers various other programs and activities for Harlem residents' health and wellness. Additionally, the Harlem Bureau houses the East and Central Harlem Neighborhood Health Action Center, a vital component of NYC's efforts to promote health equity and reduce health disparities in the neighborhood. The Bureau of Harlem Neighborhood Health is seeking a Program & Data Manager for the Creating Healthy Schools & Communities (CHSC) initiative. This role will serve as the senior programmatic and operational leader for the Harlem Bureau of Neighborhood Health's CHSC programs, which are funded by the NYS Department of Health. The program emphasizes integrated school- and community-based strategies to promote chronic disease prevention, physical activity, nutrition access, and health equity.
