About The Position

The Center for Health Equity & Community Wellness (CHECW) aims to eliminate racial and other inequities that lead to premature mortality. CHECW works to increase awareness of the harm caused by centuries of racist, socially unjust policies and advocates for redress for impacted NYC communities. They collaborate with community, faith-based, and healthcare organizations to address inequities across community and healthcare systems, focusing on social determinants of health (SDOH) to improve the well-being of New Yorkers. The Bureau of Harlem Neighborhood Health (Harlem BNH) supports this mission within its catchment area, which includes East, Central, and West Harlem, as well as Washington Heights and Inwood. Harlem BNH focuses on the root causes of health disparities and develops programs with community input, emphasizing SDOH through initiatives like the Harlem Health Advocacy Partnership and the Asthma Counselor Program. The Bureau also houses the East and Central Harlem Neighborhood Health Action Center, a vital part of NYC's efforts to promote health equity. The Bureau is seeking a Program & Data Manager for the Creating Healthy Schools & Communities (CHSC) program, a NYS Department of Health funded initiative focused on integrated school- and community-based strategies for chronic disease prevention, physical activity, nutrition access, and health equity.

Requirements

• A master's degree from an accredited college or university with a specialization in an appropriate field of physical, biological or environmental science or in public health.
• A doctorate degree from an accredited college or university with specialization in an appropriate field of physical, biological, environmental or social science and one year of full-time experience in a responsible supervisory, administrative or research capacity in the appropriate field of specialization; or
• A master's degree from an accredited college or university with specialization in an appropriate field of physical, biological, environmental or social science and three years of responsible full-time research experience in the appropriate field of specialization; or
• Education and/or experience which is equivalent to the above.
• At least a master's degree in an appropriate field of specialization and at least two years of experience.
• Two years as a City Research Scientist Level I can be substituted for the experience required.

Responsibilities

• Oversee data collection, monitoring, and reporting for programmatic, community engagement, and evaluation activities.
• Identify and disseminate best practices.
• Utilize Public Health Partners Connect and other Harlem Bureau data systems to track participation, partnerships, and outcomes.
• Integrate CHSC data with broader Harlem Bureau reporting systems.
• Prepare reports, dashboards and presentations for internal teams, funders and external partners.
• Present neighborhood health data to partners to inform shared strategies and collective action.
• Develop and maintain strong relationships with schools, community-based organizations, city and state agencies, and local community leaders.
• Build and support coalitions to leverage resources, strengthen coordination, and increase program sustainability.
• Provide training and technical assistance to partners implementing CHSC-aligned activities.
• Serve as programmatic and operational lead for day-to-day planning, implementation and monitoring of CHSC within Harlem Bureau.
• Manage all grant-related activities including budgeting, reporting, contracting, staffing and compliance.
• Translate grant deliverables into actionable workplans aligned with Bureau and community-wide priorities.
• Support program sustainability and continuous improvement efforts.
• Coordinate vendors, facilitators and partners supporting physical activity, nutrition and wellness initiatives.
• Support integrated school-community programming that reflects neighborhood needs and lived experience.
• Manage staff.
• Complete special projects and initiatives.
• Represent Bureau of Harlem Neighborhood Health at meetings as needed.
