As a Program Coordinator, you will provide operational oversight and coordination for designated service delivery areas, ensuring high-quality supports are delivered in accordance with individualized plans. You will collaborate with interdisciplinary teams, monitor service quality, support staff development, oversee documentation and compliance activities, coordinate medical and wellness needs, and help individuals achieve their goals while promoting independence, community involvement, and overall well-being.

Requirements

  • Bachelor's degree from an accredited college or university required.
  • Relevant experience in the field of Developmental Disabilities may be substituted for education on a year-for-year basis.
  • Successful completion of background screenings, including criminal history, driving record, abuse/neglect, and fingerprint checks.
  • Current driver's license, acceptable driving record, and current auto insurance required.
  • Must have a vehicle with working air conditioning and heating if used to transport clients.
  • Must have a reliable means of communication.
  • Ability to read, understand, and interpret written materials.
  • Ability to write clearly, accurately, and legibly.
  • Must be willing to participate in an on-call rotation according to the regional office schedule.

Responsibilities

  • Coordinate the development, implementation, and monitoring of Individual Support Plans (ISPs) to ensure meaningful and measurable outcomes.
  • Support individuals in achieving personal goals while promoting independence, community participation, and natural support systems.
  • Lead, mentor, supervise, and evaluate Community Service Managers, Primary Support Staff, and Direct Support Staff.
  • Collaborate with support teams, nurses, families, and other stakeholders to ensure individuals receive quality care and services.
  • Monitor the health, safety, and well-being of individuals served through regular site visits and ongoing oversight.
  • Coordinate medical services and wellness initiatives, including support for healthy lifestyles, medication oversight, and treatment compliance.
  • Ensure team members receive required training and remain knowledgeable about individual support plans and service expectations.
  • Review records, therapy documentation, and service documentation to ensure accuracy, completeness, and compliance.
  • Conduct audits and quality reviews, including documentation reviews, progress monitoring, and record assessments.
  • Maintain client records, staffing information, budgets, and related service data within organizational systems.
  • Monitor budget performance, identify variances, and assist with annual budgeting processes.
  • Ensure residential procedures, money counts, and controlled medication counts are completed appropriately when applicable.
  • Maintain compliance with organizational policies, HIPAA requirements, and regulatory standards.
  • Perform other duties as assigned.
