About The Position

The Program Coordinator, Community Supports is responsible for assisting individuals receiving supports through the Community Supports department to achieve their goals through development and implementation of their Individual Support Plan. The Program Coordinator is also responsible for providing leadership and coaching direct care staff on the skill of active treatment.

  • Ability to maintain a positive relationship with community organizations, as well as with family members and other support service providers.
  • Ability to maintain appropriate relationships with people served, families and providers, i.e. maintaining appropriate professional boundaries, and treating them with respect and dignity.
  • Ability to perform with a minimum of supervision.
  • Uses correct judgement, initiative, and flexibility to meet job requirements.
  • Ability to represent The Arc of Evansville professionally in the community by developing effective work relationships with internal and external contacts (i.e., verbally, written, attitudinal).
  • Promotes a positive relationship and attitude among others by serving as a role model; interacts in an attentive, courteous, and timely manner.
  • Must have good keyboarding skills and be able to enter in a timely manner and accurate manner all information into the Agency’s billing system, ACCEL.
  • Must have good organizational and problem-solving skills.
  • Ability to work daily with individuals served who may exhibit potentially aggressive behavior.
  • Must have access to a reliable and safe vehicle for transportation of individuals served.
  • Must be able to work nonstandard/flexible hours, including some weekend, evening, and holiday hours.
  • Must receive and maintain certification for First Aid/CPR.
  • Must receive and maintain Core A and B certification for medication administration.
  • Must receive and maintain crisis Mandt training/certification upon hire.
  • Must pass a Department of Motor Vehicles background check and hold a valid driver’s license.
  • Must have and maintain an acceptable driving record, i.e., less than 3 moving violations within 3 years.
  • Must have and maintain automobile insurance coverage as required by The Arc of Evansville’s insurance carrier or Arc policy.

  • A Bachelor level degree in a human service field is preferred.

Responsibilities

  • Conducts self professionally with truth, accuracy, fairness, and responsibility.

  • Foster and support a person-centered thinking atmosphere.
  • Works with individuals assigned to caseload on the development, implementation and review of their person-centered Individualized Program Plan and assists in training direct support staff on client needs and goals.
  • Collects and analyses data to report progress on the individual’s goals.
  • Supervises and evaluates the performance of direct reports and conducts timely counseling and disciplinary actions, if necessary.
  • Completes and submits all required documentation required to meet BDDS, Medicaid Waiver, and any other funding or regulatory source requirements.
  • Maintains files of individuals served in accordance with the agency’s Record: Maintenance, Retention and Transfer Policy and Procedures.
  • Assures that documents are filed in a timely manner.
  • Develops Residential and Community Integration schedules, training schedules and schedules for individuals served.
  • Assists with assigned vehicle schedule, upkeep, and maintenance of agency vehicles.
  • Demonstrates commitment to health and safety by leading by example and by participation on health and safety trainings, safety inspections, and the development of safety procedures.
  • Adheres to all safety rules and procedures.
  • Trains staff on specific health and safety procedures to minimize risk.
  • Corrects hazards and unsafe conditions.
  • Completes timely investigations on accidents and rapidly corrects accident causes.
  • Provides referrals for individuals on caseload for services that are needed/desired outside of the service delivery system of Arc.
  • Adheres to state regulations.
  • Maintains professional communication with parents/guardians and other service providers concerning individuals on assigned caseloads.
  • Assists in implementing behavioral management strategies per established behavior management plans.
  • Assures that individual’s rights and dignity are maintained.
  • Assures that required state and agency reports are completed thoroughly and submitted on a timely basis.
  • Assures that direct support staff are trained on individuals’ specific information.
  • Assists with staff training.
  • Trains staff and models active treatment/engagement.
  • Regularly assesses delivery of Active Engagement throughout the program area; provide training and modeling of Active Engagement in each program room; hold staff accountable for consistent delivery of Active Engagement.
  • Coordinates room activity/assignments to maintain adequate staffing/training.
  • Assists in emergency situations and in crisis prevention and intervention.
  • Provides individual and group habilitation and completes all documentation as assigned.
  • Maintains confidentiality regarding staff, individuals served and their families.
  • Ensures regular community outings for individuals are developed and executed.
  • Evaluates employees annually based on current job description roles and responsibilities.
  • Uses universal precautions and provides personal assistance, as needed, to individuals receiving services at The Arc of Evansville.
  • Regular attendance is an essential function of this job.
