About The Position

The Clinical Programs Coordinator plays a vital role in the oversight, coordination, and implementation of Roots Community Health Center's Respiratory Health clinical initiatives. This position focuses on driving health equity, reducing health disparities, and delivering targeted education, screening, and navigation services for respiratory health and infectious disease management. In addition to clinic program operations, this role is instrumental in extending critical respiratory health resources directly into the community.

Requirements

  • Minimum of 2 years of experience working in a non-profit organization or community health clinic.
  • Knowledge of, or willingness to learn, basic disease management concepts, health messaging, and clinical guidelines specific to respiratory health.
  • Demonstrated passion for eliminating health disparities and empowering underserved communities.
  • Flexibility to work weekend schedules for outreach events as required.
  • Ability to communicate clearly, empathetically, and respectfully with diverse populations, particularly low-income individuals and communities of African descent.
  • Demonstrated ability to discuss sensitive, complex, or stigmatized health conditions and behaviors with zero stigma or bias.
  • Strong organization, time management, and multitasking skills to manage program workflows alongside weekend event logistics.
  • Proficiency with Microsoft Office, G-Suite, and the ability to quickly learn Electronic Medical Record (EMR) systems and specialized databases.
  • Ability to identify gaps in care or communication and work proactively with multidisciplinary teams to resolve issues.
  • Valid California Driver’s License with a safe driving record (or ability to obtain).

Responsibilities

  • Track program and contract deliverables specifically tied to respiratory health initiatives (i.e., Asthma and COVID), ensuring all targets are met in a timely and effective manner.
  • Serve as an instrumental lead for weekend outreach events, coordinating logistics, participant engagement, and field-based health education/screening efforts.
  • Coordinate activities across patient navigation, outreach, and patient services teams to ensure seamless service delivery.
  • Communicate effectively across all levels of the organization, community partners, and funders to advance respiratory health program goals.
  • Oversee recruitment, enrollment, and targeted outreach to maximize community participation in respiratory health prevention/treatment resources.
  • Ensure thorough and consistent documentation of all program activities, immediately reporting data collection issues to the Program Administrator and Medical Director.
  • Keep clinic and navigation staff informed on program workflows, referral criteria, and continuum-of-care linkages for respiratory health patients.
  • Assist in drafting protocols, training modules, and culturally tailored health communications regarding respiratory care and infection control.
  • Identify training gaps and operational bottlenecks, contributing to clinical strategy and program innovation.
