About The Position

As a Program Clinical Coordinator, under the direction of the Program Director, you will manage the clinical integrity of the program and provide supervision to Lead Care Managers and Community Health Workers. You will also oversee compliance with key program metrics, including outcomes, documentation, and quality of service delivery, to ensure consistent, high-quality care.

Requirements

  • Licensure as a LCSW, LMFT, Psy.D, Ph.D, PCP, RN or other licensed health care professional.
  • Minimum of 2+ years of experience working with individuals with a history of mental illness in a social service/mental health environment. Depending on program/division, demonstrated knowledge of Evidence Based Practices and/or the Recovery Model of treatment.
  • Leadership and mentoring skills.
  • Working knowledge of health care services coordination and management.
  • Working knowledge of Agency policies and procedures, HIPAA, DMH, DHS, Department of Social Services, and other state, federal regulations relating to mental health services.
  • Current knowledge of care planning and case management for assigned clients.
  • The skill to present ideas, information, and viewpoints clearly, both verbally and in writing.
  • Personal computer skills include word processing, electronic health record, spreadsheets, and other related software.
  • Ability to use analysis, experience, and judgment to make effective decisions.
  • Commitment to team objectives and Didi Hirsch philosophies.
  • Ability to adapt to changing needs by acquiring new skills and knowledge.
  • Current California driver’s license and a driving record acceptable to the Agency’s insurance carrier.

Responsibilities

  • Consistently updates Program Director regarding key performance indicators, clinical concerns, and other metrics that are being monitored.
  • Identifies gaps in the clinical service model and works with Program Director to create needed policy, procedures, and trainings.
  • Designated as the clinical lead, oversees clinical efficiency and fidelity to the aligned program model(s); provides case assignment and tracks client progress in the aligned program.
  • Responsible for clinical supervision of assigned staff.
  • Guides Lead Care Managers in the development of their work plans, assigned duties, responsibilities, and scope of authority, consults with staff on areas of concern relative to clients.
  • Ensures client documentation is accurate and submitted within the established timeframe.
  • Responsible for oversight on clinical assessment elements, leading to the creation of the care plans are completed for all clients.
  • Maintains strong communication with a multi-disciplinary care team regarding client updates, changes in treatment plan, crisis, etc.
  • Act as clinical resource for multi-disciplinary care team, as needed, including taking lead on crisis intervention and case conference.
  • Provides clinical support to lead care manager where care is needed, including medical offices, community, at client’s home, etc.
  • Facilitate access to primary care and behavioral health providers, as needed to assist care coordination or multi-disciplinary team.
  • Develop and implement clinical training as needed for population focus.
  • Works closely with QA department to support documentation compliance and takes the lead in all clinical documentation internal audits.
  • Monitors the flow of cases on supervisees’ caseloads to ensure that a balanced approach to intakes and discharges occur.
  • Forms a highly effective clinical team to accomplish the program’s short- and long-term goals.
  • Develops higher levels of expertise in staff members by encouraging further education, participation in seminars and providing learning opportunities within the program.
  • Attends, participates, leads and/or facilitates routinely scheduled meetings as requested or required for the program, division, or Agency.
  • Interfaces with related community agencies, attends Health Plan’s meetings as scheduled and/or needed.
