About The Position
St. Ann’s Community is hiring a full-time Program Assistant (CNA) for the Adult Day Services Home on its Irondequoit Campus. The role performs routine procedures according to established policies and care plans. Positions are available for day and evening schedules, Monday, Tuesday, Wednesday, and Friday, with no weekends or holidays.
Requirements
- Current, valid New York State Nursing Assistant Certification.
- High school diploma or GED.
- Six months of related experience.
Responsibilities
- Bathe and groom participants and conduct toileting rounds every two hours.
- Assist participants with transfers, ambulation, positioning, and lifting.
- Assist participants with meals and supervise and encourage those who need meal cueing; complete meal nourishment documentation in the EHR, including for the Family-style Dining Program.
- Assist with recreational activities and programs throughout each program day, including transporting participants to chapel, field trips, and community-integrated events.
- Attend required annual in-service programs, including six hours of in-house in-service every six months.
- Assist with participant admissions, transfers, and discharges.
- Report changes in participants’ condition or behavior in a timely manner.
