About The Position

St. Ann’s Community is currently hiring a Program Assistant (CNA) in the Adult Day Services Home located on the Durand Campus. This individual is responsible for the delivery of care, for a specific number of participants, that meets the standard of care as directed per assignment and participant care plans. This is a temporary position with the potential to become permanent based on performance and organizational needs.

Requirements

  • Current/Valid NYS Nursing Assistant Certification required
  • High School Diploma or GED required
  • 6 Months of related experience

Responsibilities

  • Responsible for the delivery of care, for a specified number of participants, that meets the standard of care as per nurse aide assignment and care or service plans.
  • Responsible for bathing, grooming and toileting of participants and support the dining experience.
  • Performs rounds every two to three hours for toileting and change of position or as needed.
  • Provides assistance with transfers, ambulation, positioning and lifting with/without mechanical lifts.
  • Provides range of motion as required in Min-Gym weight training and ambulation program per Rehab. Recommendation.
  • Serves dietary meals, nourishments and fluids, assist participants with meals and supervises, encourages participants who requires cueing with meals, completing meal nourishment documentation in EHR, including Family-style Dining Program.
  • Under the supervision and guidance of the supervisor or recreational therapist, will assist with recreational activities and programs throughout each program day, including transports to chapel and field trips or community integrated events.
  • Takes care of participant's clothing, sorts and places soiled clothing in proper receptacle and laundered if needed.
  • Reports any change in participant's condition or behavior in a timely manner.
  • Reports incidents/accidents to Supervisor and complete witness statement when called upon to do so.
  • Completes required paperwork / EHR documentation
  • Performs simple treatments.
  • Reports suspected abuse/neglect to the Nurse or Director of the program.
  • Assists with admissions, transfers and discharge of participants.
  • Assists with orienting new participants and family members to location & routines.
  • Gives motivation and encouragement to participants.
  • Participates in Team huddles, staff meetings, and the CEO Town Hall Meetings
  • Prepares participants for appointments, medical tests, therapy & activities.
  • Maintains confidentiality of all participant care information.
  • Follows infection control procedures as directed.
  • Places participant belongings in safe locations.
  • Keeps work areas clean, clear & safe.
  • Responsible for assigned fire duties and various emergency procedures.
  • Participates in staff development programs on and off unit or site
  • Responsible for attending required annual in-service programs; (6) hours of in-house in-service every (6) months.
